Derrick etc. Dropped Objects 2015 • During rope access inspection of crown, spanner (0. 3 kg) detached from lanyard & fell 64 m to pipe deck • While running 10 -3/4” casing Topdrive Casing Alignment Tool (T-CAT) piston rod (4 kg) detached & fell 15 m to rig floor. (Diller picked up too high putting weight of casing string on single-joint elevators)

Derrick & Well Ops Dropped Objects – 2016 • Piece of angle iron (7. 75 kg) detached from draw-works HVAC ducting & fell 20 m to doghouse roof & striking glancing blow to IP’s hard hat (IP received first aid for stiff neck)

Crane & Lifting Operations - 2015 • Steel inspection hatch (3. 6 kg) fell 7. 6 m from crane air intake

Lifting Operations 2016 • Lifting LMRP, 10 ft piece of dunnage fell 7 m from LMRP to deck • Moving gravel pack engine (15 Tonne), crane main hoist wire failed; load descended 6 m to deck • During lift through hatch, steel section (14 kg) caught by tag line and fell 5. 4 m to level below.

Other Dropped Objects - 2015 • • Redundant vent pipe cowling (1. 8 kg) fell 10 m to 15 m to cellar deck in high winds • GPA warning beacon (20 kg) parted from mounting and fell 2 m to extent of cable. Combination of high wind and incorrect bolt. • • Unsecured spanner (1. 2 kg) fell through deck penetration 8 m to grating below • • • During storm, scaffold board (14 kg)worked loose & fell 16 m to level below Sheet steel insulation (2 m x 0. 25 m) believed to have fallen 30 m from exhaust stack to deck Tubular handrail (1 kg) fell 11 m to cellar deck (corroded welds) Hammer head (3. 2 kg) fell 4. 8 m to scaffold platform (hammer shaft broke) Fibreglass cover (7 kg) to helideck fire hose box detached by wind to fall 6. 8 m to walkway Section of turbine exhaust (50 kg) came adrift inn high wind & fell 3 m to deck After storm scaffold access platform under cellar deck found damaged with sections missing or hanging 15 m below

Other Dropped Objects – 2016 • 120 o section of conductor buffer (20 kg) found detached from deck, landing on scaffolding 9 m below • Hammering loose corrosion prior to cutting & removal, section of diesel pipe-work (5 kg) detached & fell 3 m • • Section of steel cladding (7 kg) from 18” pipe, detached & fell 5. 5 m • During high winds, 9 scaffold boards (18 kg each) supporting stairwell cladding dislodged and fell between 3 m & 11 m • During high winds, section of helideck panels (180 kg) came free from mounting & fell 13 m to crane pedestal • Instrument tech dislodged scaffold pole (8. 9 kg) which fell through deck penetration to deck below, having previously fallen 3 m in to mech workshop Part of turret wind wall cladding (15. 6 kg) dislodged by high wind (74 kt gusting to 97 kt); blown directly into sea.
