Mandatory Health Insurance • Health insurance is mandatory for all Pace University students. Waivers • Pace University will consider waiving (removing) the fee for Health Insurance only if the student demonstrates coverage under another insurance policy, whose coverage is determined to be equal to or better than the coverage provided under the University’s policy (as specified in the brochure). – Note: The non-University insurance policy would not necessarily provide coverage for Emergency Medical Evacuation and Repatriation of Remains. However, by signing a waiver form, the student acknowledges their understanding and acceptance that – in cases where the student waives University health insurance for a policy that does not cover Emergency Medical Evacuation and Repatriation of Remains – the student alone (not the University) will be solely and exclusively responsible for any and all such expenses. • Waiver forms are available here: http: //www. pace. edu/healthinsurance/sites/pace. edu. healthinsurance/files/Pace%20 International%20 Waiver%201314. pdf

Waivers • When you complete the waiver form please return it to: The Allen J. Flood Companies, Inc. Two Madison Avenue Larchmont, NY 10538 attn: Pace International Student Plan Please include a copy of your other insurance policy in English and in U. S. dollars. You can also email a copy of the waiver form and policy to pace@ajfusa. com.

Waivers • Waivers should be submitted no later than June 10, 2014 for students enrolling in the Summer 2014 semester.

United. Healthcare Student Resources Website • Insured students have online access to claims statuses, EOBs (explanation of benefits), ID Cards, network providers, correspondence and coverage information by logging in to My Account at: www. uhcsr. com/myaccount • Students who have not yet set up their online account should click the “Create My Account Now” link, then follow the onscreen directions to establish an online account using the email address on file (or 7 -digit Insurance ID number if you have received it).

Insurance ID Cards • Insurance ID cards are no longer automatically mailed out to students. Instead, an email notification will be sent to the student when the digital ID card is available to be downloaded from My Account. An insured student may also use My Account to request delivery of a permanent ID card through the mail. • ID Cards may also be accessed via the mobile site at: my. uhcsr. com

In Case of Emergency • In case of an emergency, students should go to their nearest hospital or healthcare location! • If at all possible, in a non-emergency situation, it is best for students to go first to Pace University’s Health Care Center. • If the student is not in the area, it is recommended that the student search for a healthcare center which is covered by the student’s University insurance (use the “Search for Provider” function on https: //www. uhcsr. com/pace).

Pace University Health Care Center • The University Health Care Center is able to provide students with basic care, as well as to provide referrals if the student requires additional treatment not offered by the Health Care Center. • The Health Care Center is located at 41 Park Row on the 3 rd Floor and may be reached by telephone at (212) 346 -1600.
