Examples PHP 1 (place & community) Trusts look after a range of community assets that have a significant impact on providing safe places that nurture and promote health and wellbeing for the population e. g. : community and leisure centres, libraries, parks, pitches, learning centres, visitor attractions PHP 2 (early years) • Trusts play an important part in the wholesystems approach • Provide opportunities for parents to maintain healthy lifestyles from pre-conception onwards • Provide activities and classes that support health and wellbeing at low or no cost • Music tuition in schools and extra-curricular activities. PHP 3 (mental wellbeing) • Suicide prevention training • Wide range of services and activities enable people to engage in the “ 5 ways to wellbeing”: connect; be active; be curious; learn; give (e. g. adult learning, outdoor activities, sports development) • Part of CPP and sub groups e. g. HLH on Mental Wellbeing Sub Group, supporting whole system approach PHP 5 (poverty & inequality) • Budget leisure membership: 50 p for people accessing certain benefits • Bespoke programmes with DWP e. g. supporting people in into employment • Bespoke programmes for people with long term conditions such as Type 2 Diabetes, Cancer and Cardio Vascular Disease Adult Learning | Archives | Arts | Leisure Facilities | Libraries | Museums| Music |Sport & Outdoor | Youth Work
