1. Introduction Symtech is committed to providing a safe and healthy work environment for all employees, subcontractors, and visitors. Symtech expects all employees, subcontractors, and visitors to fully comply with Occupational Safety and Health Act administered by the Occupational Safety and Health Administration (OSHA) as minimum standards. It is Symtech’s goal to ensure each worker leaves at the end of the work day, as physically fit and sound as when they began. 2. Health and Safety Policy Statement Symtech considers Health and Safety to be of primary importance and recognizes the right of every employee to work in a safe and healthy work environment. We will operate in a manner that will promote and protect the Health and Safety of those working for us. As per of our safety culture, our health and safety policy statement will be reviewed annually with all employees within our establishment. 3. Workplace Violence and Harassment Policy Statement – Symtech is committed to the prevention of workplace violence and harassment and promotes a violence and harassment free workplace. Symtech has a zero tolerance for those committing acts of violence and/or harassment, as such any violence or harassment committed by or against any member of our workplace or public will not be tolerated and will result in disciplinary action (depending on the severity of the act). Symtech is committed to investigating reported incidents and complaints of workplace violence and harassment in a fair and timely manner, taking the necessary action to respond to those events, and providing support for complainants. All incidents and complaints of workplace violence and harassment must be reported to your supervisor or manager immediately. © Symtech 2018 6

Responsibilities, Accountabilities & Legislation The Internal Responsibility System (IRS) is a safety philosophy based on the principle that everyone in the workplace is responsible for his or her safety and for the safety of co-workers. Ø Establish and maintain a joint health and safety committee, or cause workers to select at least one health and safety representative. Ø Train employees about any potential hazards and in how to safely use, handle, store and dispose of hazardous substances and how to handle emergencies. Ø Supply and maintain personal protective equipment and ensure workers know how to use the equipment safely and properly. Ø Immediately report all critical injuries to the government department responsible for OSHA. Ø Appoint a competent supervisor who sets the standards for performance, and who ensures safe working conditions are always observed. © Symtech 2018 8

Responsibilities, Accountabilities & Legislation All workers must: Ø Work in compliance with the OSHA ØUse or wear the equipment, protective devices, or clothing required by the employer Ø Report to his or her employer or supervisor about any known missing or defective equipment or protective device that may endanger himself, herself or another worker Ø Report to his or her employer or supervisor about any known workplace hazards or violations of the OSHA Ø Not remove or making ineffective any protective device required by the employer or by the regulations © Symtech 2018 11

Responsibilities, Accountabilities & Legislation No worker shall: Ø Remove or make ineffective any protective device required by the regulations or by his or her employer, without providing an adequate temporary protective device. When the need for removing or making ineffective the protective device has ceased, the protective device shall be replaced immediately Ø Use or operate any equipment, machine, device or thing or work in a manner that may endanger himself, herself or any other worker Ø Engage in any prank, contest, feat of strength, unnecessary running or rough and boisterous conduct © Symtech 2018 13
4. Responsibilities, Accountabilities & Legislation Occupational Safety and Health Administration (OSHA) ØAll employees working at Symtech must comply with the OSHA standards as a minimum standard. ØSymtech has responsibility to provide a safe workplace. Symtech MUST provide their worker with a workplace that does not have serious hazards and must follow all OSHA safety and health standards. ØSymtech must prominently display the official OSHA Job Safety and Health – It’s the Law poster. This poster describes rights and responsibilities under the OSH Act. ØIn addition, Symtech will post OSHA citations and injury and illness data where workers can see them. ØSymtech will notify OSHA within 8 hours of a workplace fatality or within 24 hour of any workrelated inpatient hospitalization, amputation or loss of an eye. © Symtech 2018 14
5. Safety Violation Disciplinary Policy Type 1 – Situations which may cause self injury. Ex: Failure to wear PPE, blocking a guard on a tool, climbing the wrong side of the ladder or verbal violent remarks/gestures/actions will result in the following steps: Ø 1 st violation – verbal, documented reprimand Ø 2 nd violation – written reprimand suspension without pay Ø 3 rd violation – termination with cause Type 2 – Situations that present an imminent danger. Ex: Failure to wear body harness, lanyard, lockout on electrical systems, Sexual harassment (verbal) with/without witness or sexual harassment (physical) without a witness will result in the following steps: Ø 1 st violation – upon observation, stop operation at once and move employee to safe environment. Written reprimand issued / possible suspension without pay. Ø 2 nd violation – termination with cause Type 3 – Situations in which employee knowingly bypasses a safety device or creates a situation which may seriously or fatally injure another person. Also, physical violence and/or physical sexual harassment with a witness will also result in the following steps: Ø 1 st violation – termination © Symtech 2018 15
6. Personal Protective Equipment Symtech will provide training to each employee who is required to use PPE. Employees shall be trained to know at least the following: a) when PPE is necessary; b) what PPE is necessary; c) how to properly don, doff, adjust and wear PPE: d) the limitation of the PPE; and, e) the proper care, maintenance, useful life, and disposal of the PPE © Symtech 2018 16
7. Fall Protection Ø Symtech shall provide a training program for each employee who might be exposed to fall hazard. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Ø Symtech shall assure that each employee has been training, as necessary, by a competent person. Ø Symtech will verify compliance of this training record(s). Ø When required, retraining will be provided to those who demonstrate a lack of understanding and/or skills required under this requirement (Subpart M – Fall Protection) © Symtech 2018 17
8. Accident/Incident Reporting Ø Symtech is required to notify OSHA when an employee is killed on the job or suffers a workrelated hospitalization, amputation, or loss of an eye. Ø A fatality must be reported within 8 hours. Ø An in-patient hospitalization, amputation, or eye loss must be reported within 24 hours. ØOSHA defines in-patient hospitalization as a formal admission to the in-patient service of a hospital or clinic for care or treatment. Ø Reports can be made in person (nearest OSHA office), by telephone (1 -800 -321 -OSHA) or online using Serious Event Reporting Online Form found at www. osha. gov. ØTo ensure consistency of information and a timely submission, reports should only to be submitted by the supervisor or Head Office. © Symtech 2018 18
9. Accident/Incident Investigation After an incident occurs, an investigation will begin and the following steps will be conducted: ØTake control of the situation ØProvide first aid or comfort to injured worker. ØAttain name of hospital if applicable. ØSecure and isolate the incident scene. Ensure nothing is touched or disturbed within the scene. Tape off the area immediately. ØAdvise Head office of the incident ØGather witnesses and get statements. Take photos of incident scene. ØComplete the Incident/Accident Report and submit the report to Head Office Ø Look at every part of the accident scene. Write down as much detail as you can. Ø Determine the primary and secondary causes of the incident. ØFollow up with injured worker ØStart Return to Work program if applicable © Symtech 2018 19
10. Return to Work ØWorkers must report all work-related injuries/illnesses as soon as possible. ØSymtech is committed to preventing injuries and illnesses in the work place by maintaining a healthy and safe work environment. ØAn important part of this commitment is to assist any employee who has been injured or who becomes ill as a result of workplace incident by returning him/her to work in a safe manner. ØAll workplace parties have a duty to cooperate. ØSymtech will provide modified duties (verbal and documented) for all types of injuries © Symtech 2018 20

1. General Site and Company Rules The following company rules apply to all employees and subcontractors. Failure to follow company rules may result in disciplinary actions that could lead to termination of employment or contract. Ø Employees are representatives of Symtech. As such, you are required to conduct yourself accordingly. Ø Employees are expected to arrive at work on time and ready to work. In the event you may be late or absent, notify your supervisor as soon as possible. Ø Any requests for time off work must be completed in advance and approved by your supervisor Ø Personal work or activities may not be conducted during working hours. Ø Workers may not deliberately tamper/damage any work equipment, vehicles and tools. Workers must report any defects in equipment, tools and vehicles immediately to a supervisor. Ø Do NOT operate ANY motorized vehicle or equipment while on the phone © Symtech 2018 22
1. General Site and Company Rules (Cont. . ) Ø All employees shall follow the OSHA safety and health standards and any safe work practices and/or procedures, at all times. Ø All employees shall conduct themselves in a professional and respectful manner towards other employees, supervisors, management and customers. Ø Acts of workplace violence, harassment or threats are not acceptable in the workplace. Ø No employees shall consume or distribute alcohol and/or narcotics in the workplace. Ø Criminal activities (such as theft, vandalism, public indecency etc. ) are strictly prohibited, and will result in immediate dismissal. Ø Smoking is NOT permitted in the workplace Ø All employees shall report an unsafe act or hazardous situation immediately to a supervisor. © Symtech 2018 23
2. Emergency Procedures and Plan All personnel will be trained in site-specific emergency response procedures and will follow these procedures when required. All personnel shall be familiar with the location of fire extinguishers and first aid kits. A copy of the plan will be kept within the workplace and available to employees for review. In the event of an emergency, all workers are to evacuate the office immediately and report the incident to their supervisor. A muster point will be determined by the supervisor and communicated with all employees. A review of the emergency action plan will be conducted when: 1) The plan is developed or the employee is assigned initially to a job 2) The employee’s responsibilities under the plan change; and 3) The plan is changed. © Symtech 2018 24
3. Site Safety Documentation Weekly Safety Talks A safety talk is a hands-on way to remind workers that health and safety is important on the job. The supervisor will be required at least once per week to conduct a safety talk with all of the workers. The safety talk will discuss health and safety related topics that are relevant to the job task or scope of work being performed. All workers will have to attend and sign off that they have participated and understood the safety talk given. Once completed, a copy of the safety talk will be forwarded to Symtech Innovations head office to be kept on file. © Symtech 2018 25
3. OSHA-Hazard Communication Standard (HCS) The purpose of hazard communication training is to ensure that information regarding hazardous materials is effectively communicated to all employees in the workplace through labeling, materials safety data sheets (MSDS)/safety data sheets (SDS), and training. Each employee who handles, works with, or works in close proximity to a hazardous material or has potential for exposure shall be trained on HCS. All MSDS / SDS for products being used in the workplace will be maintained at each work location and be made available upon request. In the event that a supplier label is missing or damaged, a workplace label should be created and applied to the product. In the event of a chemical spill report it immediately to your supervisor. © Symtech 2018 26
