Path was launched in 2012 through support from The Clinton Health Access Initiative (CHAI) and in cooperation with NASTAD § Mission: Provide a streamlined process that ensures timely access to life-saving medications for uninsured people living with complex, chronic conditions such as HIV/AIDS and hepatitis C. § Vision: People living with life-threatening illnesses can obtain all necessary medications with ease and without barriers. § Proprietary online portal was created in support of Harbor. Path’s Mission and Vision
Harbor. Path Introduction § Pharmaceutical manufacturers provide medications through the Harbor. Path Portal at no charge to a patient. These include: Abb. Vie Gilead Sciences Johnson & Johnson Patient Assistance Foundation, Inc. Merck Vii. V Healthcare
Harbor. Path Programs – PAP Harbor. Path provides medications for those who are: – Uninsured – Experiencing a gap in health insurance coverage – Ineligible for ADAP assistance in their state of residence • An example: denied ADAP for Over Scale – Pending approval for health insurance coverage or ADAP The Online Process • • • Case managers apply for HIV/AIDS, hepatitis C or chronic medications through the Harbor. Path portal; One application is used for multiple medications. Eligibility determination instantaneous Medications are shipped from the mail-order pharmacy within 48 hours of approval, direct to patient or healthcare facility Refills done online or by phone Proven patient health benefits (Adherence studies) Nationwide- the portal program is available throughout the U. S.

Short-Term Safety-Net Program Through a coordinated effort with the Texas State ADAP, Harbor. Path is offering a state-wide short term safety-net program for uninsured patients waiting for ADAP approval. The goal of this program is to fill a broader need which aligns with the mission of Harbor. Path
