Survey • Should fit on ONE or TWO full pages • Start off with ‘Please complete …’ and ends with ‘Thank you for completing …. ’ • Begins with question to find most used topic • Then focuses on ONE of the topics • Space or line divides the two sections of the survey • Information must be quantifiable, i. e. be put into a chart/graph • Question and options need different indenting, options with greater indenting
