Background Detail • Found good practice across most care areas • Areas of concern included - Harvington mental health ward related to visibility of patients and ligature risks - Minor issues with medicines management - MCA training shortfall - Delays in access to CAMH’s and mental health services

Transforming Care – Post Winterbourne • The ‘Transforming Care’ agenda continues to be a high priority for the locally and nationally • The CCG, in partnership with the council, are committed to delivering the outcomes of the Transforming Care Agenda • All patients with a Learning Disability/Autism/behaviours that challenge within an in-patient setting for three months or longer are subject to a Clinical Treatment Review, this is a process to determine whether the right approach is being taken by clinical teams to support discharge from hospital • The process has led to many safe and appropriate discharges of these vulnerable patients to less restrictive environments

Urgent Care and Patient Flow within the county • SRG have approved the system wide improvement plan • Our PMO is responsible for overseeing delivery of the full plan • The Acute Trust have dedicated actions to improve flow throughout the hospital and are undertaking a “breaking the cycle” event from 6 TH July to commence transformation

Planned outcomes for “Breaking the cycle” • Improved processes for flow of patients within the acute trust • Continued support from partners to maintain performance in relation to Delayed transfers of care (DTOCs) • Designed to achieve improved patient experience of urgent care and patient flow and to improve performance related to urgent care
