Good Practice • All commissioned services accept referrals via one route • SPE hosted by Coventry and Warwickshire Partnership NHS Trust. • Referrals screened by clinician/practitioners who make a decision re most appropriate service across tiers • Clinicians/practitioners across 4 tiers • Self referrals are accepted • Professional referrals are accepted • Step up and down across tiers

Early Intervention & Building Resilience
Targeted support for Looked After (LAC) adopted children and young people, their parents/carers, and associated professionals (e. g. Social Workers). Services and support for children and young people with low to moderate emotional wellbeing. Targeted tier 2 Service – Vulnerable/at risk Children

Reach Model • Counselling: Reach will provide counselling to c&yp using their preferred method of delivered. Either Face to Face or via • Therapeutic Group Work: Programmes include; Low Mood, Anger Awareness, Anxiety Management, Separation & Loss and Bereavement. Peer support: Opportunity for c&yp to manage their issues through peer support via: Online forums & Group sessions • Self-help resources / information Signposting and linking to other services • Online self-help resources • Online information
The staff team Journeys and Reach employ a team with a wide range of skills and qualifications including: • Primary Mental Health Workers • Occupational Therapists • Counsellors • Youth workers • NNEB • Psychologists (outsourced)

• Access to support before crisis point – making sure people with mental health problems can get help 24 hours a day and that when they ask for help, they are taken seriously. • Urgent and emergency access to crisis care – making sure that a mental health crisis is treated with the same urgency as a physical health emergency. • Quality of treatment and care when in crisis – making sure that people are treated with dignity and respect, in a therapeutic environment. • Recovery and staying well – preventing future crises by making sure people are referred to appropriate services.
