General Practice Clinical Pharmacist GPCP Competency and Capability
General Practice Clinical Pharmacist (GPCP) Competency and Capability Framework: Learning from the pilot assessment Reid F, Stewart F, Macdonald A. Background Scottish Government funded 140 WTE pharmacists to support GP practices between 2015 -2018. NHS Education for Scotland (NES) designed a Learning Pathway for these pharmacists including a GPCP Competency and Capability Framework - developed to describe knowledge, skills and attributes needed to perform the GPCP role safely and effectively and support ongoing clinical development. The Framework is based on the Royal Pharmaceutical Society (RPS) Faculty Advanced Practice Framework with a focus on domains relating to ‘expert professional practice’ and ‘collaborative working relationships’. Aim TURAS Portfolio is used to record evidence of performance. To implement a system to credential GPCPs and to learn from this process. Do • Each portfolio reviewed independently by 2 external assessors (n=8) • Candidates invited for viva voce (n=6) • Portfolios calibrated by external assessors, moderated by NES panel chair • Gaps in evidence against competencies identified • 4 standard questions developed based on framework competency themes • Candidates asked 4 standard questions plus 4 individualised questions based on gaps identified • 45 minute viva voce. Panel – NES chair plus 2 external assessors who reviewed portfolio • Debrief session – 4 external assessors and NES Study Plan • Review assessment processes within NES and externally • Agree and develop assessment system (portfolio assessment and viva voce (interview)) • Develop assessment tools • Identify external assessors ( RPS Faculty credentials plus GP practice expertise) • Invite GPCPs to participate in pilot assessment • • Candidates consulting with patients in one therapeutic area as Independent Prescriber (IP) Good reflections on applying learning to practice Positive, effective interactions with patients and practice staff Variety of evidence recorded to meet competencies Lack of triangulation of evidence- observations/ feedback from other clinicians and patients Case studies lacked relevant clinical information Evidence linkage against competencies to be more specific Requirement for guidance on portfolio submission Act Biannual assessment Commissioned research on fit of Framework to Pharmacotherapy service ‘Guidance for Submission of GPCP Portfolio’ agreed by NES and external assessors 1) Mandatory certificates for IP qualification, consultation skills and core clinical assessment skills course completion 2) Triangulation of evidence from Supervised Learning Events (mini-CEX and Case-based Discussions), Patient Satisfaction Questionnaires and peer review of recorded patient consultations 3) Case studies to include full structured clinical history 4) Evidence to be linked to maximum of 4 competencies • Request for all candidates to submit further evidence to meet newly defined minimum requirements → All completed in 3 month period and gained accreditation • Guidance distributed to GPCPs and their educational supervisors • Promotion of guidance at territorial Health Board events References 1.
