Match Record vs. Report • A match report is a form with the game significant details, submitted to the appropriate authorities • The information from the game record is used to complete the match report

He fell awkwardly and hit his head on the ground. Play was stopped and the Blue coach came on the field. The player was substituted and did not return. After the game, I checked with the coach, who said #12 had a bad headache and was sensitive to light. His mother took him to the hospital.
