Food as Medicine Clinic Targeted intervention addressing food insecurity in context of chronic disease. • Therapeutic food pantry + education and access to RD • Choice pantry model • Feeding the whole household • 2 visits per month for 6 months, up to a year if needed • Assessing outcomes (pilot)
Target Population (Pilot Phase) Participant criteria: • Positive screen for food insecurity during hospitalization or Internal Medicine clinic visit. • Ongoing relationship with Metro. Health Primary Care Provider • One or more of the following diagnoses: o Uncontrolled diabetes o Uncontrolled hypertension o Recent worsening of heart failure
Once a participant is referred… Participant reports to pantry during open hours FAM Supervisor Ellen Mc. Laughlin demonstrates a pantry visit experience. • Intake form completed ancillary services offered. • Participant “shops” for 3 days worth of healthful foods for their household • Staff assists with shopping, bagging groceries; offers parking voucher or bus pass • Participant returns up to two times per month for up to 12 months • Nutrition information, recipes, nutrition counseling offered each time
Participants choose foods from each food group. • Shelf Stable foods • Fresh produce, eggs • Frozen meats and fruits
Where we are now Preliminary measures Participants say the program helps: “Better eating available on limited income. ” “When I’m short on food it helps my family. ” • 63 households served • 168 people • 20% children • 24% seniors (60+) • Trend toward increased intake of fruit and vegetables, less fast food • Majority report improved access to healthy foods.
Produce Distribution at Metro. Health Janet Montoya, Mgr. Community Health Advocacy • Started in June; 3 rd Tuesday of the month • 464 households served in 2 months • 1305 people • 255 seniors impacted Thanks to WRAAA for the canvas bags! Participants love them.
