Fee Waiver Program Employee Eligibility • Full-time permanent, probationary or temporary employees • Permanent part-time employees • Employees on an approved leave of absence, if otherwise eligible • Temporary faculty with three (3) year appointments and coaches with at least six (6) years full-time equivalent service. • FERP employees are eligible for fee waiver only during the quarters when they are actively employed. Fee Waiver Program Employee Fee Waiver – how to apply? • Enrolling in Work Related/Career Development Courses • Not required to matriculate into a degree program. • Complete a paper CSU undergraduate or postbaccalaureate/graduate application. • Go to HR for an eligibility verification form before taking paper application to Admissions. Fee Waiver Program Enrolling in Baccalaureate Degree Program? • If you plan to matriculate into a baccalaureate degree program (i. e. BA, MS), you must use the online application. • You must apply during the application periods for new undergraduates or post baccalaureates; check for deadlines on the Admissions website and review the selection requirements. Fee Waiver Program Employee Fee Waiver – how to apply? • The Admissions Office will notify you when your application has been processed and you have been admitted to the university. • Complete the Individual Career Development Plan, obtain the required approvals, and submit the form to Human Resources after being admitted to the University. Fee Waiver Program Employee Fee Waiver – Registration • Register for classes online via CPReg. • Complete the Employee Fee Waiver Course Notification HR 133 form, obtain the required approvals, and submit the form to Human Resources before the start of EACH QUARTER. • Rule of thumb – turn in your forms when you register. Fee Waiver Program Guidelines • Fees are waived for a maximum of two (2) classes, or 6 units, whichever is greater. • If the unit/course limitation is exceeded, the employee must pay the difference in fees. • All classes must be taken for academic credit. • Classes may NOT be audited. • Fees cannot be waived for courses taken through Continuing Education. Fee Waiver Program Dependent Fee Waiver eligibility criteria: • CSUEU (Units 2, 5, 7, and 9) SETC Unit 6 Spouse/domestic partner* or dependent child under age 25 • All other eligible personnel - Spouse/domestic partner* or dependent child under age 23 *Requires employee to have a declaration of Domestic Partnership on file with the California Secretary of State. Fee Waiver Program A dependent child is defined as: • Your child or stepchild who has never been married • A child living with you in a parent-child relationship who is economically dependent upon you and has never been married • Requires completion of the affidavit of Eligibility for Economically Dependent Children Form. • Your child or stepchild who is incapable of self-support due to a disability that existed prior to age 23. • Requires completion of the Disabled Dependent Benefit Form. Fee Waiver Program How to Apply? • Your dependent must apply during the CSU application periods for new undergraduates or post baccalaureates. • Dependents may use the transferred benefit at any other CSU campus. • Check for deadlines on Admissions website and review the selection requirements. Fee Waiver Program Intercampus Employee Dependent • Dependents fall into three categories: • Cal Poly employee dependent attending Cal Poly • Cal Poly employee dependent attending another CSU • Another CSU employee’s dependent attending Cal Poly • Dependent Fee Waiver forms, must be submitted for each quarter or semester.
