MAJOR HAZARDS • Identified 7 Major Hazards within our operations: • (those hazards we know will cause fatalities on the mine) 1. Falls of Ground 2. Trackless Mobile Machines 3. Conveyor Belts 4. Electricity and Isolation 5. Ventilation 6. Winches and Rigging 7. Track bound Machinery
MAJOR HAZARD MANAGEMENT PLAN (MHMP) • Inter-departmental effort MHMP • Team Leader for each MHMP • Champion for each site • Leading indicators • Monthly audits of MHMP by Site Champions • FOG Data base kept by Team Leader • Annual audits by Team Leader • ICAM investigations done on all incidents relating to MHMP
KEY LEARNINGS • Should not be an additional procedure. • Should pull the key controls together into a single document. • Must be regularly audited to give assurance of the current controls. • Must produce leading indicators (triggers). • Must assign responsibility for everyone involved in the MHMP. • Must establish a rigid structure of reporting/communication for each site.
FALLS OF GROUND: key controls Reporting • – Hazard/Q-Rating and GPR (Actual vs. Planned) – Planned Task Observations (Actual vs. Planned) – % substandard support and of pillars robbed in a section. – Panels stopped by TARP (High Risk Panels) – Fall of Ground Register – Controlled and uncontrolled falls of ground – Frequency of falls and Fall out sizes – Fall out thickness Review • – Review of the risk assessment every 6 months – MHMP key controls reviewed once a year or after incidents by review committee.

WHY WAS TARP INTRODUCED? • Multiple fatalities at Maloma colliery. • Fatality at Kroondal Mine. • A serious injury at Helena Mine. • Several near misses.
DEVELOPMENT OF THE TARP SYSTEM • Involvement of Experienced Mining Personnel and Strata control Experts. • Standardizing Geotechnical and Geological Terminology. • Involve the training department at all phases • Development of learning materials i. e. Training( VRT)
T. A. R. P. - Overview 1. System devised to dictate a level of response to underground conditions. 2. Ground conditions will determine the task team required. 3. Strategy to minimize rock falls and part of MHMP for ground control. 4. Three rating conditions used: Normal - Green Level 1 - Yellow Level 2 - Red Risk Based approach and escalates the decision making hierarchy
