Extended Primary Care Team • EPCT provides a service in Fareham, Gosport and SE areas and supports older people with complex needs to better manage their overall health and wellbeing • Care Planners and Health Connectors work alongside one another to address the health and social needs of individuals • EPCT is a multi-disciplinary ‘social prescribing’ service and this approach leads to a range of positive health and wellbeing outcomes for people, as well as contributing to reducing the use of NHS services.

EPCT Achievements • Working with patients in this way has achieved a reduction in contacts with primary care, calls to 111/999, and unplanned admissions to hospital • The multi-disciplinary approach enhances integrated working across health, social care and the community and voluntary sector • During lockdown, in addition to usual business, the team supported people who needed urgent help with getting food shopping and collecting medication
