EOEA Transitional Plan – In Person Visits • Elder Affairs has been working on a transitional plan for in-person visits • Released to the network 8/6/2020 • Guidance applicable across all EOEA Programs • Programmatic Considerations & Interim Actions are still in effect – Assess each consumer’s situation & determine if an in-person visit is appropriate 10

Discussion • Long Term Placement: – [HC & CSSM] Placement in a nursing facility and no longer working towards discharge to a community setting • Rehab/Nursing Facility Stay: – Consumer is in a Rehab/Nursing Facility beyond 90 days and efforts for discharge planning have not yielded a discharge date. Refer to CSSM/Options Counseling for tracking 14
Mass. Health Nursing Facility Sanctions • Mass. Health has sanctioned 3 nursing facilities: NFs notified on Monday, August 3, 2020 • NFs have 30 days to respond • MH members & authorized representatives will be notified by mail • Notices mailed on 8/4/2020 • 3 Nursing Facilities sanctioned are: – Hermitage Healthcare – Worcester • Elder Services of Worcester Area – Town & Country Healthcare – Lowell • Elder Services of Merrimack Valley & North Shore – Wareham Health Care – Wareham • Old Colony Elder Services 15

Health Nursing Facility Sanctions • No ASAP action is currently required – however ASAPs may begin to see transfer screening requests & requests for assistance with discharge planning • Please track requests for transfer screenings – Mass. ADH Updates • Elder Affairs will share ADH Bulletins & Rate Regulations with ASAPs as the information becomes available • ADH will receive a rate increase as of August • Ability for members to attend: – in-person and/or – remotely • Remote ADH engagements limited to 3 x/week 18
ADH Updates • OLTSS updating Elder Affairs on the status of ADH re-openings every Wednesday • As of 8/5/2020: • 59 ADHs have sent attestations to OLTSS • 45 ADHs open as of 8/5/2020 • 14 ADHs plan to open by the end of August • OLTSS tracking closures of ADH Programs • As of 8/5/2020: • 3 ADHs have closed • 1 ADH are slated to close in September • 3 ADHs are slated to close in October 19
