Evaluation Web New Interface • Evaluation Web will be going offline for updates from Adobe Flash to HTML 5 • Tuesday, 12/17/2019 @ 6 pm EST until Friday, 12/27/2019 @7 am EST • Jurisdictions will receive 72 hour notice from Luther Consulting • New System only supports Internet Explorer (Version 11), Google Chrome, and Firefox • New System will not include Non-Testing (Risk Reduction Activities) Module • Users log-in will remain the same & authorized users can only access Eval. Web via SAMS. • Clear browser cookies and cache prior to logging in

The form will have the following sections to fill out: Evaluation Web New Interface HIV Testing Form • General • Client Information • Testing Information (Negative/Positive Testing Information) • Additional Tests • Pr. EP Awareness & Priority Populations • Essential Support Services (i. e. health benefits & risk-reduction intervention) • Local Use Fields You must make a selection for each field in the General section before you can proceed to the next section of the form.
Reflexx will not be available after the system updates; users will not be able to access their saved reports Reports New canned reports will be available by January 2020. New Report Builder will be available in Summer 2020. Users can access previously entered testing data via reports function, however, current reports are limited to a data extract. Users will enter the date parameters and export all variables via CSV format. In this format users can filter data, generate counts, etc.
