ETA Emergency Incidents- 11/18 Date Incident Area Status 11/8/18 Fire B 90 Break Room Small fire started inside microwave oven. Investigation showed that recycled napkin to be the source (small metal fragments in dye). Significant Safety Achievements- 11/18 • • The time sensitive chemical self-assessment report has been completed and is pending final signatures prior to submittal. Arrangements were finalized for the set-up of a Class 4 laser experiment in the B 70 hallway. A Temporary Work Authorization was issued by the LSO. All ETA office personnel were assigned EHS 0260 “Basic Electrical Safety” training. We started with around 400 assignments and only have 100 remaining. The hydrogen sensors in 70 -257 received their annual calibration and testing. A number of old gas cylinders were identified behind Building 64 and returned to various vendors. An ETA Safety Alert was issued regarding breakroom microwave oven safety precautions. All calcium gluconate HF gel stations staged in the B 62 and B 70 lab areas were swaped out prior to expiration.
Short Term Safety Outlook 11/18 • • Finalize procedures and Work Activity for the flammable refrigerant project in the B 60 air conditioner test chambers. Start the lab area seismic protection self-assessment project. A new ESDR Lab Safety Lead meeting has been established. We will be meeting as a group on a quarterly basis to discuss issues they need to be aware of. Lab Area Safety Lead hands-on training will be implemented. Continue efforts at improving the procurement process for NRTL listing of electronic equipment. Continue old chemical clean-outs in ETA lab areas. Building-wide cleanouts are being planned for early 2019. We will be seeing a surge in home telecommute ergonomic assessments in December due to the annual sign-up cycle. Ensure lab areas prepare for the holiday shutdown.
