Completed Electrical Shock 5/23/19 ESDR 70 -279 Worker suspected strain from frequent torqueing on pressure reactors. Further investigation identified work station set-up as primary cause. Worker touched the top of an ozone generator equipment and received a minor 2 milliamp shock. First Aid Equipment evaluated and no obvious problems, but there is electrical leakage when the equipment is initially turned on. Tagged out of service. Further follow-up needed regarding awareness of timely reporting.

Significant Safety Achievements- 5/19 • The new ”Electrical Equipment Procurement Guide” has been finalized

Significant Safety Achievements- 5/19 • The new ”Electrical Equipment Procurement Guide” has been finalized and made available to ETA personnel. I have also been sharing this document with other interested LBNL divisions. • Provided follow-up to the electrical shock incident in 70 -279. A component inside an ozone generator has failed. It caused a low level 2 milliamp shock to a researcher. • An ETA Safety Alert was issued for Bicycle Safety Awareness. This includes transportation modes that are prohibited at LBNL. • Annual updates/renewals of Level 3 Work Activities continues. • We had a large influx of new personnel during the month. A great deal of effort went into ensuring required training was completed. • Clean-up of 62 -220 continues. Old/abandoned tubing and equipment was salvaged. General clutter being reduced. More to come. • I completed my 8 -hour Hazardous Waste Operations and Emergency Response (Hazwoper) renewal training.

Short Term Safety Outlook 6/19 • Complete the Lab Area Safety Lead self-assessment project.

Short Term Safety Outlook 6/19 • Complete the Lab Area Safety Lead self-assessment project. • Complete an annual update of the ETA Integrated Safety Management (ISM) Plan. • Develop guidelines for purchase and operation of microwave ovens, lab ovens/furnaces, and hot plates used in lab areas. This is a project for the LBNL Laboratory Safety Committee. • Continue work on completing telecommute home office ergonomic assessments. • Follow-up on the numerous moves taking place in the B 51 F, B 63, and B 64 areas. Some waste disposal is needed. Door placards, etc. need to be updated. • Provide support for the new Cyclotron Road cohort. This includes development of new work activities and lab area set-up. • Provide follow-up on worker awareness regarding timely reporting of suspected shock incidents to Health Services. In addition, this will include more awareness about general emergency notification procedures (calling 911 or X 6999).
