Berkeley Fire Department response. FLEXLABB 90 X Fire quickly extinguished by worker and equipment shut-off into a safe state. The “ 911” emergency number was called. ORPS issued.

Significant Safety Achievements- 3/18 • • The new ETA 0010 “ETA New Hire Safety

Significant Safety Achievements- 3/18 • • The new ETA 0010 “ETA New Hire Safety Orientation” class is ready. The course is being added to the EHS training system and Activity Manager. ETA Safety Alerts were issued for: – Emergency Notification Procedures (911) – Bicycle Safety • • • Performed electrical equipment inspections for a number of items located in 64 -224. This is an area we were not aware of. A number of old chemicals from 70 -108 were disposed. Assisted with quarterly management safety walkthroughs conducted by the Cyclotron Road and EAEI divisions. Implemented the “time sensitive chemical” controls in ETA lab areas. Chemicals were identified and labeled. A tracking log sheet was also developed. I served as a judge in the Contra Costa Science Fair. I judged a number of environmental projects designed by local high school students. I attended the annual ASSE safety symposium in San Ramon.

Short Term Safety Outlook 4/18 • • • Participate in the safety stand down for ESDR has scheduled for April 10. Start the lab area seismic protection self-assessment project. Start conducting the new ETA-0010 “New Hire Safety Orientation” class. Assist with the set-up of the new Splat Quencher and XPS equipment in 70 -108. Complete the quarterly Satellite Accumulation Area (SAA) hazardous waste inspections. Complete the annual review and update of the ETA Integrated Safety Management (ISM) Plan. Assist with the clean-out/transfer of chemicals from CYR groups leaving the Potter lab areas. Continue follow-up on repair of conditionally accepted electrical equipment. Attend the Semiconductor Environmental Safety and Health Association (SESHA) conference in Scottsdale, AZ. I will be serving as a speaker and SESHA Fellow.
