ETA Emergency Incidents- 6/19 Date Incident Area Status 6/5/19 Fire 90 -3145 DVD drive in office cubicle started smoking. A 12 volt connector had melted and caught fire. Flames observed inside DVD drive and 911 called. Fire Department response. No injuries or damage.

Significant Safety Achievements- 6/19 • Provided follow-up to the DVD drive fire incident. The

Significant Safety Achievements- 6/19 • Provided follow-up to the DVD drive fire incident. The cause of the fire was quickly identified and communicated to stakeholders throughout LBNL. A new emergency reporting “ 911” poster was developed and is being distributed throughout ETA training materials for new hires and lab workers were updated to include the “ 911” poster. • The draft report for the “Lab Area Safety Lead Self-Assessment” project was completed and submitted for review/comment. • A quarterly Lab Area Safety Lead meeting was held. The meeting was well attended and there was excellent discussion from the group on safety issues they are working on. • Annual updates/renewals of Level 3 and Level 2 Work Activities continues. • Several new Work Activities were developed for the incoming Cyclotron Road cohorts. • Clean-up of 62 -220 continues. Old/abandoned tubing and equipment was salvaged. General clutter being reduced. More to come regarding chemical disposal.

Short Term Safety Outlook 7/19 • Submit the final “Lab Area Safety Lead Self-Assessment”

Short Term Safety Outlook 7/19 • Submit the final “Lab Area Safety Lead Self-Assessment” report to OCA. • Continue efforts to increase awareness about general emergency notification procedures (call 911). Permanent signs are being prepared for conference rooms and other common areas. • Complete an annual update of the ETA Integrated Safety Management (ISM) Plan. • Develop guidelines for purchase and operation of microwave ovens, lab ovens/furnaces, and hot plates used in lab areas. This is a project for the LBNL Laboratory Safety Committee. • Follow-up on the numerous moves taking place in the B 51 F, B 63, and B 46 areas. Some waste disposal is needed. Door placards, etc. need to be updated. • A large chemical clean-out is being planned for 62 -220 and 62 -310. • Finally get the remaining 8 sulfur hexafluoride gas cylinders removed from the B 70 cage area. This has been a long and time consuming trek!
