COVID-19 and asthma: take home messages Well-controlled asthma is not associated with an increased risk of (severe) COVID-19. Use of inhaled corticosteroids (ICS) in subjects with asthma prevents exacerbations, is associated with reduced expression of ACE 2 in sputum, and should be continued during the COVID pandemic. Smoking increases the risk of severe COVID-19, and is associated with increased expression of ACE 2 in airways and lungs. The COVID-19 pandemic impacts (severe) asthma care. Antiviral drugs and immunomodulatory treatments need to be tested in randomized controlled trials (RCT).

COVID-19: clinical presentation Watch for symptoms People with COVID-19 have a wide range of symptoms – ranging from mild symptoms to severe illness. These symptoms may appear 2 -14 days after exposure to the virus: • Fever • Cough • Shortness of breath or difficulty breathing; • Chills • Repeated shaking with chills; • Muscle pain • Headache • Sore throat • New loss of taste or smell.
COVID-19: emergency warning signs When to Seek Medical Attention? If you have any of these emergency warning signs* for COVID-19 get medical attention immediately: • Trouble breathing • Persistent pain or pressure in the chest • New confusion or inability to arouse • Bluish lips or face *This list is not all inclusive. COVID-19 and asthma treatment Advise patients with asthma to continue taking their prescribed asthma medications, particularly inhaled corticosteroids (ICS). Asthma medications should be continued as usual. Stopping ICS often leads to potentially dangerous worsening of asthma. Smoking cessation! For patients with severe asthma: Ø Ø continue biologic therapy, and provide training to patients to self-administer (or homecare) do not suddenly stop OCS if prescribed.

COVID-19 and asthma action plans Make sure that all patients have a written asthma action plan with instructions about: – Increasing controller and reliever medication when asthma worsens – Taking a short course of OCS for acute asthma exacerbations if clinically indicated – When to seek medical help.
COVID-19 and asthma inhalers Avoid nebulizers where possible – Nebulizers increase the risk of disseminating virus to other patients AND to health care professionals. – Pressurized metered dose inhaler (p. MDI) via a large spacer is the preferred treatment during severe exacerbations, with a mouthpiece or tightly fitting face mask if required.

For patients with asthma, viral respiratory tract infections can have a profound effect on the expression of disease or loss of control.
Among infectious agents, human rhinoviruses are. the most prevalent in regard to asthma exacerbations. Coronavirus has not a higher frequency in AAE than control group
In a study of 49 adults presenting to the emergencies with AAE Coronavirus was found in 4% of induced sputum samples. Coronavirus seem to have minor contribution to AAE.
No asthmatic patient was identified in this report.
A total of 549 patients with COVID-19 were enrolled, of whom 548 cases were included in the study. The prevalence of asthma in COVID-19 patients was only 0. 9% It has been speculate that Th 2 immune response in asthmatic patients could counter the inflammation process induced by Co. V-2 infection.

To report findings of two complete autopsies of severe acute respiratory syndrome coronavirus 2 positive individuals who died in Oklahoma (United Statees) in March 2020. There was interstitial chronic inflammation composed mainly of lymphocytes. No eosinophils or neutrophils were identified. Sequestration of eosinophils does not seem occorring in post mortem analysis
Hypotesis The decrease of blood eosinophils (eosinopenia) in COVID is due to secundary hemophagocytosis: due to the cytokine storm (IL-6, IL-1 and TNF) in severe COVID. Macrophages are too strongly activated and eat (phagocytose) eosinophils and neutrophils In contrast, lymphocytes and monocytes decrease in the blood in COVID due to massive migration into the lungs.

The purpose of this guideline is to maximise the safety of adults and children with severe asthma during the COVID-19 pandemic, while protecting staff from infection. The recommendations are based on evidence and expert opinion.
Comunicate with patients, help to alleviate any anxiety and fear they may have about COVID-19
Tell all patients or their parent or carer, to continue taking their regular medicines Tell patients to continue using inhaled corticosteroids because stopping can increased the risk of asthma exacerbation. If the patient usually attends hospital to have biological treatments, think about if they can be trained to selfadminister, or could be treated at a community clinic or at home
Tell patients not to share their inhalers and devices with anyone else Tell patients or their parent ora carer to clean equipment such as spacers and peak flow meter regularly
Only carry out Bronchoscopy and most Pulmonary Function Tests for urgent cases and if the results will have a direct impact on patient care, because these tests have the potential to spread COVID-19.
Minimise face-to-face contact If patients are having a face-to-face appointment, on the day of the appointment first screen them by telephone to make sure they have not developed symptons of COVID-19 Ask patients to attend appointments with no more than 1 family member Minimise time in the waiting area
