MODEL FOR ENHANCED PRIMARY MENTAL HEALTH CARE SERVICES GPs MH Services Professionals Voluntary Community Sector (VCS) GPs will have support from: - • • SINGLE POINT OF ACCESS (SPA) Mental Health Professional in Primary Care Member of staff from the CMHT who will liaise Case discussion with Consultants Direct book for assessment appointments via SPA SECONDARY CARE SERVICES 1: 1 and work TALKING THERAPIES (WHM) • 1: 1 and group work • NICE Recommended psychological intervention • • MENTAL HEALTH PROFESSIONAL IN PRIMARY CARE (WHM) (IAPT complaint) CBT Primary Care Psychology Counselling Supported c. CBT • Support for step up/step down • Frontline mental health assessment • Liaison with CVS statutory and non-statutory • Advice including: : • Self help materials, access to c. CBT and signposting available to the Wellbeing Hub Public and patients Self referrals For complex patients: - • Peer support/pathway navigator • MH Professional in primary care • Access to wellbeing resources via wellbeing hub phone or website WELL BEING HUB COMMUNITY SUPPORT AND WELLBEING SERVICE • Ability to book directly into step 2 c. CBT and VCS • • • provision Eg Mood. Master Mentoring Mental Health First Aid Recovery College Social Prescribing Books on Prescription Peer Support/pathway navigation • MH awareness raising and up scaling • Mental Health Professional in Primary care SOCIAL INCLUSION support to VCS/Statutory and non-statutory services • Mental Health training directory • CPD recognition to MH training • Advice available wellbeing hub/SPA/MH Professional ENHANCED PRIMARY CARE MENTAL HEALTH SERVICE

WHM – remit • A Countywide Primary Care Mental Health Service; • People aged

WHM – remit • A Countywide Primary Care Mental Health Service; • People aged 16 and over; • Experiencing common mental health problems e. g. stress, low mood anxiety; • Provide a range of information, interventions and therapy, both 1: 1 and in a group format; • Aim to help people improve their mental wellbeing through a range of support. • All clinicians are professionally trained, registered and are receiving regular supervision.
