Immuuncompetente patiënten • Primo-infectie – “Therefore EBV serology has the major task to differentiate between acute EBV infection and acute infections with CMV, HIV, HAV, HBV, HCV, rubella virus and mumps virus. ” Clin. Immuuncompetente patiënten • EBNA-1 Ig. G antistoffen: – Detectie rond 4 -6 weken na primo-infectie – Positief resultaat wijst op vroegere blootstelling • “Anti-EBNA-1 is missing during the first four weeks after the onset of disease. Its positivity therefore allows a rather clear exclusion of acute EBV infection. ” Clin. Lab. 2001; 47: 223 -230 – Negatief resultaat: • • • Geen EBV drager Primo-infectie Vroegere infectie (5%) Immuunsuppressie met secundair verlies CAEBV

Immuuncompetente patiënten • “A reliable test for EBNA antibodies can be used as a screening test since the presence of EBNA antibodies excludes primary EBV infection. Antibodies against other EBV antigens must then be analyzed only if EBNA antibodies are absent” J. Clin. Microbiol. 1997; 35(11): 2728 -2732 • Biotest Anti-EBV EBNA Ig. G ELISA – Sensitiviteit – Specificteit – PPW 97% 100% 99, 8%

Immuuncompetente patiënten • Seroprevalentie – “In West-Europa en de USA bereikt de prevalentie van EBVantistoffen een plateau van > 95% rond 20 jaar met incidentiepieken tussen 1 en 6 jaar en tussen 14 en 20 jaar. ” Wegwijzer in Microbiologie, Editie 2000 – “Een groot deel van de bevolking wordt tijdens de kinderjaren of adolescentie besmet het EBV. Meer dan 97% van alle mensen boven de dertig jaar heeft een infectie doorgemaakt en is seropositief voor EBV. ” RIVM rapport 605148010 – september 2001: 150/253 EBV aanvragen >30 jaar (59. 3%)

Eindconclusie • Gelet op: – Antistofverloop bij primo-infectie (na 4 -6 weken verschijnen EBNA-1 Ig. G As) – Klinische vraag (primo-infectie? ) – Hoge seroprevalentie (>95% rond 30 jaar) • Kan besloten worden dat: – Het vernieuwde schema een vergelijkbare tot betere EBV diagnostiek geeft (oa. geen problemen met polyclonale respons) – 60 -80 % van de stalen slechts 1 test (EBNA-1 Ig. G As detectie) vereisen voor adequaat beantwoorden van de klinische vraag. (september 2001: 18, 5% EBNA-1 Ig. G negatief)

Immuungecompromitteerd ptn. • Immuungecompromitteerde patiënten: – Primaire immunodeficiëntie (Tcell deficiëntie) – Secundaire immunodeficiëntie • Orgaan en beenmerg transplant patiënten • HIV patiënten • Klinische vraag: – Primo-infectie/reactivatie in het kader van lymfoproliferatieve aandoeningen

Immuungecompromitteerde ptn. • Serologie? – “The laboratory diagnosis of primary and reactivated EBV infection is based on serologic methods in immunocompetent patients. However, in immunocompromised patients, serologic data are difficult to interpret and do not often correlate with clinical data. ” J. Med. Virol. 2001; 65: 348 -357 – “…we observed delayed EBV-specific Ig. M and Ig. G response compared to EBV DNA detection, indicating serology to be an insufficient late marker for EBV infection and unsuitable for PTLD diagnosis. ” Blood 2001; 97(5): 1165 -1171

Immuungecompromitteerde ptn. • PTLD – “Early identification of patients at risk for developing PTLD could reduce PTLD-related morbidity and mortality, thereby improving overall patient management. ” Blood; 97(5): 1165 -1171 – “It is now clear that a crucial point of PTLD diagnosis and management is certainly close monitoring for EBV infection. Although specific serology can be useful for the diagnosis of EBV primary infection, there are major limits in sensitivity and specificty in comparison to PCR, which lead to a reduction in its usefulness. Its only indication is probably pretransplant EBV status determination. The cornerstone of EBV infection diagnosis is the EBV viral load. ” Pediatr. Transplantation 2002; 6: 280 -287

Eindconclusie “EBV viral load determination by quantitative polymerase chain reaction (PCR) is so far the only predictive marker for PTLD prevention and PTLD treatment monitoring, although limited in specificity. ” Pediatr. Transplantation 2002; 6 : 280 -287
