We’ll start out with the links. It’s important to note that we are NO LONGER using Microsoft Excel. These links will be sent to you by your regional staff, and they are also available on our website. Please note that there are TWO DIFFERENT forms for sexual assault and domestic violence.
We’ll start with Domestic Violence. Once you click on the link, you will be taken to this webpage. You will notice that the reporting period and reporting deadlines are listed. Please note this reminder to DOUBLE CHECK your data. It will be sent back to you if there are errors.
Scroll down to see the questions. The red asterisk by each question means it is required. You will not be allowed to move to the next section unless you have answered it.
For “Reporting Period Start & End Dates, ” either type the dates in the format shown, or click on the calendar icons to select the dates on the calendar.

The next section asks you to report the racial demographics of your clients. You will notice that the answer field says, “The value must be a number. ” This means that you CANNOT enter characters that are not numbers. If you do not have any data for a particular question, please enter “ 0” – you will not be allowed to leave it blank. Please double and triple check that your numbers add up to the same number you reported in question #8 (Total Clients). If they do not, you will have to re-report your data.
Again, please double check that all the numbers you entered into this section add up to be the same as the total number of clients you reported in Section 1. If they do not match, you will have to re-report your data. The “Total” question is for you to re-calculate your answers in this section and ensure it matches with your Total Clients from Section 1. Once you are sure that your numbers are correct, click “Next” to move onto the next section.

If you do not have data for a given question, please enter “ 0” – you will not be allowed to leave the question blank. The next section is “Services Provided. ” Note that services may be provided inperson or virtually. Please note the descriptions under each question for clarification.
The next section is “Number of Support Groups Provided. ” Please note the instructions on this question: do NOT count the number of people attending here.
The next question is “Number of Support Group Participants. ” This is where you count the number of participants of support groups.
The next question is regarding trainings or presentations that your agency has provided. Please note the instructions to differentiate between the questions.
The last section asks about Shelter Services – starting with the ages of people who were sheltered by your agency.
“Age: Total” should be the total number of individuals sheltered by your agency.
Once you have answered all of the questions, please double check your data. You may even click the “Back” button to review your answers before submitting it. This will save everyone time down the line. When you are ready, you may click “Submit”. It will immediately send your data, and you will not be able to go back and change your information, so do not click this until you are ready.
