+ Course Selection Form n There are three parts to the front of the form: 1. Name and Contact Information 2. Graduation Checklist – you require a minimum of 20 courses to graduate from high school 3. Course Requests including Alternate Elective Requests and which course, if any, you are planning to take in summer school n On the back of the form is a list of the courses and their codes. n You are not allowed to repeat a course at Dr. Charles Best
