Introduction Health disparities continue to grow within the community and the city of Detroit, Michigan would benefit from a wellness center targeting vulnerable populations such as the homeless and uninsured. An environmental scan provides strong evidence to support the community wanting and needing access to health promotion services. Figure 2. Homeless(Michigan Radio, 2014)

By joining Discovering Life, a nurse is not only giving back to the community and developing a standard of care that will make a lifelong impact on those in need, but also fulfilling the idea and vision of what nursing was founded upon. Requirements: Associate or Bachelors degree in Nursing with licensure to practice in the State of Michigan. Minimum 2 years of clinical experience. Specialties preferred. Duties include: • Assures quality of care by adhering to therapeutic standards. • Conducts patient assessments, administers medications, documents in a timely and accurate manner, and provides follow-up care. • Measures health outcomes against patient care goals and standards; makes or recommends necessary adjustments. • Follows the community center and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations. • Promotes patient independence by establishing patient care goals. • Teaches patient and family to understand their condition, medications, self-care skills, and answers all follow-up questions. Conclusion Vulnerable populations, such as the homeless, experience significant disparities in access to and the use of healthcare services, life expectancy, morbidity, and mortality (Martins, 2013). Investing in the vision of Discover Life Community Wellness Center will create a new hope in the city of Detroit for health services. The proposed plan for the community center is attainable and sustainable with the help of investors funding. The proposed plan figures in needed health services, revenue resources, and a budget realistic to garnered support. A community that comes together, thrives together.
