Documentation • Disability Documentation Form – This form can be obtained on the Disability Page of the JJC Website or can be picked up in the Disability Office. – The Disability Documentation Form is to be completed by a licensed healthcare professional to support the disability – The Disability Documentation Form can be completed by a qualified treating or diagnosing healthcare professional (psychologist, psychiatrist, therapist, licensed certified social worker, medical doctor, optometrists, etc. ) who is not a family member of the student
Documentation • Letter from Health Care Professional – Healthcare professionals can provide a written letter with a diagnostic statement identifying the disability, the date of diagnosis and recommendations for accommodations – The letter must be on letterhead, typed, signed by the credentialed professional and must include name and title of professional, license number, address, phone and signature of professional. – For deaf or hard of hearing students, an audiological report from a licensed audiologist (or professional) verifying the degree of hearing sensitivity would be the preferred documentation. An IEP or 504 plan could also be considered for documentation purposes.
Documentation • An Individualized Education Plan (IEP) or 504 – Students can request their IEP or 504 from their high school to provide the Disability Services office. These records do not automatically transfer with the students' transcripts. – Disability Services requests the three year re-evaluation and/or psychological report. This is usually done during the student's sophomore or junior year of high school. – Both the IEP and the 504 should have the following information: • Diagnosis • Explanation of academic, emotional or social obstacles • Accommodations used in high school
Intake Meeting What to expect: • What are your strengths? • What accommodations do you use? • What are your challenges? • What are you concerns with transitioning?
