Diabetes prevalence • It is estimated that over 5 million people in England are at risk of developing Type 2 diabetes • If current trends persist, one in ten people will develop Type 2 diabetes by 2034 • Since 1996 the number of people with diabetes in the UK has more than doubles from 1. 4 million to 3. 3 million • Type 2 diabetes accounts for at least 90% of all cases of diabetes
Healthier You • Healthier You: NHS Diabetes Prevention programme has been developed collaboratively by NHS England, Public Health England Diabetes UK • Up to 100, 000 places will be made available, across the country by 2020 • The London Diabetes Prevention programme has started to accept referrals with 624 people being supported in Year 1, 1, 378 in Year 2 and 459 in Year 3
Eligibility criteria • 18 years old and over • Registered with a GP Practice in one of the following: – – – Hounslow Ealing Westminster Kensington & Chelsea Hammersmith & Fulham • Referral sent from GP practise or NHS Health check provider • Hb. A 1 c between 42 -47 mmol/mol (6. 0%-6. 4%) or Fasting Plasma Glucose between 5. 5 -6. 9 mmols/l • Not pregnant
Programme content • Nutrition for health • How to incorporate physical activity into your lifestyle • How to solve problems that can get in the way of healthy changes • How to develop the skills and knowledge to maintain these changes after completing the programme • Facilitation by a local Health &Wellbeing Coach
Programme commitment • 1 to 1 initial assessment • 10 x weekly group sessions that includes healthy lifestyle advice and physical activity (2 hours each session) • 4 x monthly groups sessions that include strategies to help you maintain your lifestyle changes • 3 x one to one progress review sessions at 3, 6 and 9 months • Regular weigh-ins (where applicable) and Hb. A 1 c blood tests to review progress.
Next steps • Local delivery venues identified - evening and weekend as well as weekday sessions to meet the needs of a range of different people. • Referrals accepted from Mid August • Group delivery starts Mid September
