Why TAY? Children and Family Services 4, 223 children were in out-of-home care as of June 30, 2014 • 582 were ages 16 to 18 • 190 were ages 19 and older • 303 youth currently in Extended Foster Care (AB 12) as of July 7, 2015 • Receive Support until age 21 Probation 9, 438 TAY were in Probations system in FY 14/15 Homeless In 2014, there were over 36, 886 homeless public school students in San Bernardino • 9, 955 from Grade Levels 9 -12 Hospitalizations In 2013 2, 906 children/youth were hospitalized for Mental Health Issues Department of Behavioral Health www. TAY Centers Overview Transitional Age Youth (TAY) Centers provide integrated services to unserved, underserved and inappropriately served youth ages 16 to 25 with emotional and/or behavioral issues. • Goals: Independence, reduce hospitalizations or higher level of care, reduce involvement in criminal justice system and reduce homelessness. • TAY Centers: Drop-in Services/Outpatient Mental Health Services – No cost – Peer Driven – Countywide: Ontario, Victorville, Yucca Valley • Multi-Disciplinary Teams ü Clinic Supervisor ü Psychiatrist ü Clinical Therapist ü Social Worker ü AOD Counselor ü Peer and Family Advocate Department of Behavioral Health www. Crisis Residential Treatment (CRT) Program Voluntary CRT Center for TAY ages 18 to 25 in need of higher level of care than outpatient mental health but lower than a psychiatric hospital. • Peer Driven • 14 Bed • Services 24 hours a day, 365 days a year • 30 to 90 day stay • Co-ed Services include: • • Individual and Group Therapy Crisis Intervention Medication Support Drug and alcohol counseling/referrals Prevocational preparation Rehabilitation/Recovery (daily living skills) Wellness Recovery Action Plans Prerelease/discharge preparation and planning Department of Behavioral Health www. TAY Outcomes Youth Served FY 13/14 TAY System of Care provided services to over 900 TAY youth at risk and/or with mental health issues access appropriate services. 88 TAY were diverted from psychiatric hospitalization and/or higher levels of care by accessing TAY CRT (The STAY) services. DBH & TAY Program Funding The TAY System of Care is funded by a combination of Mental Health Services Act (MHSA), Medi-cal Federal Financial Participation and 2011 Realignment funding. • One Stop TAY Centers are funded by the MHSA Community Services and Supports (CSS) component. The estimated program MHSA expenditure for FY 15/16 is $5, 799. • The TAY CRT (The STAY) is funded by the MHSA Innovation (INN) component.
