Dementia (post diagnostic support) Dec 2016 Update This Months Progress Objectives for the dementia post diagnostic programme 1. Scope the current access to and interventions by 5 of the allied health professions after diagnosis. 2. Implement a change programme in 2 NHS boards/local authority areas, with the objective to enhance self management and access to AHP services 3. Complete literature review of AHP contribution to post diagnostic support 4. Scope current training in post diagnostic support 5. Embed the role of allied health professionals and post diagnostic support to evidence based policy document commitment 4 (Scotlands Dementia strategy ‘Connecting People Connecting Support’ 1. Testing of DLW Framework and use of Measurement Tool completed in Dumfries & Galloway and Edinburgh. Data being analysed and meeting with AILIP Improvement Support (Mark Hamilton) arranged in January 2. Focus meetings with DLWs involved in Improvement work being arranged for New Year. 3. Ongoing collation of AHP Self Management resources, and population of online platform. Connecting DLWs with information/resources on request. Information Gap analysis underway. 4. Continuing to align work with Focus on Dementia PDS work programme 5. Connecting People Connecting Support: OT Home Based Memory Rehabilitation National roll out progressing with 11 Health Boards involved Risk/barriers/decision making 6. AHP dementia policy ‘Connecting People, Connecting Support’ remains on target and this work will be integrated into the AILIP and a supporting document for Dementia 3 due February 2017.
