Discuss Foster Parent Consideration Staffing Have Provide Discuss the foster parent(s) option to adopt a child that has been placed in their home for at least 12 months Have the foster parent(s) acknowledge the discussion by signing the Foster Parent Affidavit for Consideration of Adopting Foster Child(ren) Currently in Home. Provide the foster family with Foster Parent(s) Notification of Decision Regarding Interest in Adopting Foster Child(ren) (form 151) and inform them of seven-day timeframe for submission.
a. Within 30 Days of the request from DFCS compile and provide verification that the foster parent(s) meet the basic requirement of an adoptive resource family. Adoption Consideration Evaluation b. Conduct a joint face-to-face visit with CPA foster parent(s) to gather the information to assist in completing the adoption consideration Narrative. c. Provide a written recommendation regarding the foster parent’s capacity to meet the short and longterm needs of the child(ren). d. If the decision is to not approve the home a face to face meeting should be conducted with the foster parent.
Adoption Consideration Evaluation Narrative Can also be used to reassess the caregiver(s) ability to continue to meet the needs of the child(ren) any time there is a CPS investigation, policy violation(s) or changes in family composition.
