Introduction § Benefits for eligible CUPE education workers will be provided through the CUPE EWBT – an employee life and health trust (ELHT) § Ontario government will fund the CUPE EWBT via negotiated amount per FTE (full-time equivalent) employee, plus a reserve § CUPE EWBT will comprise more than 40, 000 members § Actives § Eligible retirees Our new benefits plan starts March 1, 2018 for permanent employees, as well as those casual or temporary employees previously eligible under their Collective Agreement. Retirees will transition no later than May 1, 2018. 3

Life coverage Board, insurer or administrator Establish initial supplemental life or optional life coverage available under the CUPE EWBT without evidence of insurability 5. Health/Dental coverage Insurer or administrator Determine current level of Health/Dental coverage 6. Dependant information Insurer Capture details on covered dependants (names, birthdates, etc. ) 7. “Not at work” Board Confirm if member is on Life waiver of premium, or confirm current Life amounts if no waiver is in place 8. After completing your enrolment § Members who select health coverage will have their benefits cards mailed to them. Card(s) should arrive within 10 business days after enrolment is completed. § Register for Group. Net for Plan Members, to submit and inquire about claims status and history for any claims incurred on or after March 1, 2018. 19
