UCan. Go 2. org Visit UCan. Go 2. org to print the FSA ID Worksheet. The worksheet is also available in Spanish. As you set up your FSA ID, you can record your information here for future reference. Be sure to keep the worksheet in a safe place.

The alternate phone number is optional. Enter your complete address. Choose preferred language--English or Spanish.

Challenge Questions Choose four different challenge questions and enter each of your answers under the corresponding question. If preferred, you may check ‘Show Text’ to ensure accuracy.
