What is a Care Coalition? ü Purpose • To formally organize players with a community-level focus to reduce hospital readmissions of Medicare beneficiaries in local hospitals by enabling safe and effective transitions of patients between all settings of care, and empowering patients and their families to manage their health. • National Health Care Improvement Initiative The Centers for Medicare & Medicaid Services (CMS) reports that Quality Improvement Organizations (QIOs) have helped to save nearl $1 billion by improving care transitions across the country.

What are the elements for a Successful Care Coalition? ü Goals • Organize a forum for key health care providers of the community to meet • • • Conduct analysis to identify the root cause of need (RCA) Engage in social determinants such as education in self-management of health Establish evidenced based interventions to assist in improving processes or systems (per RCA) Monitor and track interventions Work in collaboration with local agencies and area stakeholders towards a common goal ü Assessment (RCA) • • • Conduct needs assessment of healthcare process Out of 22, 678 discharges in Texas, 34. 1% were readmitted within 7 days WHY? Factors: Do patients have in-home support? Needed medications? Follow up visits with physician? WHY NOT?

The Texoma Care Coalition ü Purpose • To improve the health of our community, thereby reducing hospital readmission rates in our community • To be a voice for the patient and family • Assist the patient’s ability to self-manage their own health through patient-centered and patient directed models of care • To foster wise stewardship of limited community resources, reducing redundancy, waste, and build and sustain a community coalition with a focus on improving transitions of care for Medicare beneficiaries • To improve patient safety during transitions between health care settings • To enhance the satisfaction of providers and staff through increased collaboration • To collaborate and encourage efforts of organizations with shared visions and public policies that furthers the vision
