Performing Risk Assessments What is a risk assessment? • • Defined as “an assessment that examines a process in detail including sequencing of events, actual and potential risks, and failure or points of vulnerability and that prioritizes, through a logical process, areas for improvement based on the actual or potential impact (that is, criticality) of care, treatment, or services provided. ” A proactive risk assessment is encouraged when there is potential process vulnerability or high-risk processes/actions/events that could result in a poor outcomes and/or there is no prescriptive guidance or regulation. Risk Assessment Template Step 1: Identify Potential Risks Are there any hazards or process vulnerabilities? What is likelihood of harm? Step 2: Determine Scope Who may be harmed by the issue/action and how? Step 3: Evaluate risks and determine risk mitigations (precautions) What is likelihood a hazard will occur and how severe will the consequence be? Can interventions be put in place to reduce the level of risk? Step 4: Decide, document, & take action Decide if process will proceed. If yes, ensure risk mitigations are in place prior to implementation.
