Surveillance �Professional responsible for surveillance activities is trained in basic epidemiology, surveillance and IPC �IPC team has sufficient time (but no more than 30%) to perform surveillance activities �Objectives and priorities of surveillance are defined and aligned with national program � Surveillance is conducted with active data collection methods and standardized case definitions � Surveillance data is analyzed and disseminated to all interested parties

Monitoring & Evaluation �Well‐defined M&E plan with clear goals, targets and operational plans exists �Tools to collect information needed for M&E in a systematic way developed �M&E indicators are defined �HCF regularly reports on the state of the IPC goals and strategies and the impact of the IPC activities �Evaluation of the performance of local IPC programs is performed in a blame free institutional culture

Links With Public Health & Other Services �Procedures for the links between HCF and public health services are defined �Links with other services providing health care (HC) are established �Links with other services not under Mo. H (but related to HC and response to emergencies) are established �IPC elements integrated into the national general emergencies preparedness plans �The IPC program is involved in coordination of response to public‐health emergencies
