Core Competencies Nicholas M. Barbaro, M. D. UCSF
Why do we have to do this? • Because the RRC says we do • Because the ACGME tells the RRC it has to do it
Why is this important? • Represents a standardized set of guidelines wherein all residents and all residency training programs can be evaluated • Specifies “core” components the training and evaluation of training • Your Program will be evaluated by these standards
What are the competencies?
Core Competencies 1 • • Patient Care that is compassionate, appropriate, and effective for the treatment of health problems and the promotion of health Medical Knowledge about established and evolving biomedical, clinical, and cognate (e. g. epidemiological and social-behavioral) sciences and the application of this knowledge to patient care
Core Competencies 2 • Practice-Based Learning and Improvement that involves investigation and evaluation of their own patient care, appraisal and assimilation of scientific evidence, and improvements in patient care • Interpersonal and Communication Skills that result in effective information exchange and teaming with patients, their families, and other health professionals
Core Competencies 3 • • Professionalism, as manifested through a commitment to carrying out professional responsibilities, adherence to ethical principles, and sensitivity to a diverse patient population Systems-Based Practice, as manifested by actions that demonstrate an awareness of and responsiveness to the larger context and system of health care and the ability to effectively call on system resources to provide care that is of optimal value

Technical Competencies
Research rotations
Where to use them • Evaluation forms – By rotation – At conferences – 360 – Technical competencies • In the PIF – Most PIFs already have all the documentation in them, but are not organized by competency
V. Professionalism • Describe at least one learning activity, other than lecture, by which residents develop a commitment to carrying out professional responsibilities and an adherence to ethical principles. • How does the program promote professional behavior by the residents and faculty? • How are lapses in these behaviors addressed?
Systems-Based Practice • Describe the learning activity(ies) through which residents achieve competence in the elements of systems-based practice: work effectively in various health care delivery settings and systems, coordinate patient care within the health care system; incorporate considerations of cost-containment and risk-benefit analysis in patient care; advocate for quality patient care and optimal patient care systems; and work in interprofessional teams to enhance patient safety and care quality. • Describe an activity that fulfills the requirement for experiential learning in identifying system errors.
Core Competencies Summary • They are with us so learn to love them • Useful to review your program with competencies in mind – Improve on educational aspects of rotations and other aspects with competencies in mind – Evaluate for “missing” components • Review PIF in advance to have examples of how competencies are a part of your training • Good luck!
