Chronic bronchitis (만성 기관지염) u 특별한 원인 질환 없이 객담을 동반한 기침이 일년에 3개월 이상, 연속적으로 2년 이상 지속되는 것 - clinical Dx. - simple chronic bronchitis, chronic mucopurulent bronchitis, chronic obstructive bronchitis, chronic asthmatic bronchitis

Emphysema(폐기종) u Terminal bronchioles 이하의 air space distension과 alveolar wall의 파괴 - 대개 조직 또는 CT 소견으로 진단 (pathologic Dx. ) - centriacinar(m/c), panacinar, distal acinar - a 1 -antitrypsin과 관련(panlobular)

COPD Risk factor • • • Smoking(m/i) - COPD환자의 90%이상에서 흡연력 있음 Air pollution, dusts & gases – SO 2, NO 2. . Environmental/occupational factor Infection – viruses, mycoplasma, bacteria. . Familial & genetic factor - a 1 -antitrypsin(AT) deficiency -> emphysema

COPD pathophysiology u u u u Airflow limitation -> EPP(equal pressure point)가 small airway쪽에 존재 Hyperinflation -> dynamic inflation Impaired gas exchange : small airway narrowing -> V/Q mismatch -> hypoxemia Pulmonary hypertension -> RVH & failure Secondary erythrocytosis Renal & hormonal dysfunction Cachexia -> calorie intake failure

COPD severity stage Ⅰ(mild) FEV 1(predicted) ≥ 80% Ⅱ(moderate) ⅡA ⅡB Ⅲ(severe) 50 ~ 80% 35 ~ 50% ≤ 30% Stage ⅡB -> acute exacerbation 위험 증가 Stage Ⅲ -> Life quality 심하게 저하.

COPD treatment - Medication • Bronchodilators - anticholinergics(ipratropium bromide: Atrovent) -> B 2 -agonist보다 효과적, 부작용 적다 -> 투여후 30 -90분 최대 효과, 4 -6시간 지속 - short-acting B 2 -agonist aerosol(albuterol, terbutaline) -> anticholinergics 치료에도 호전없을때 or stage 1에서 간헐적 호흡곤란시 사용 - long-acting theophylline(aminophylline) -> weak bronchodilator, 호흡근육 기능 촉진 -> 운동능력과 mucociliary clearance 증가

COPD treatment - Medication • Glucocorticoids - 천식 때처럼 효과적이지는 않음 - Dz. Severity와 입원의 필요성 감소시킴 - indication(asthmatic or allergic component 있을 때) -> 호흡곤란이 2 -3주씩 심해졌다 완화됨(asthmatic com. ) -> 질병의 reversibility가 가능하다고 생각되면 : bronchodilator의 투여로 FEV 1이 20%이상 증가시 -> 청진시 wheezing 들릴시 -> sputum or blood eosinophilia -> atopy의 증거 - COPD의 급성 악화의 빈도는 감소시키지 못함

COPD treatment - Medication u Stage Ⅰ(FEV 1 > 50%) -> short-acting B 2 -agonist prn. u Stage Ⅱ(FEV 1 35~49%) -> anticholinergics + short-acting B 2 agonist u Stage Ⅲ(FEV 1 < 35%) -> + long-acting B 2 agonist or theophylline 추가

COPD treatment - Oxygen q 저농도 산소요법의 효과 - q pul. HTN, RHF ↓(∵ pul. Vasoconstriction ↓) exercise tolerance 향상 secondary polycythemia 교정 체중 증가 life span 연장 : long term survival 증가시키는 것은 오직 O 2와 금연뿐 방법 - Pa O 2 60 mm. Hg(Sa. O 2 90%)이상 유지(휴식, 취침, 운동시 모두 ) - 하루중 많은 시간 하는 것이 효과적 (15 hr 이상)

u 산소요법의 문제점 - oxygen toxicity : free radical 관련 -> 모세혈관 누출, 폐포 출혈, surfactant dysfuntion, 폐부종 발생 - CO 2 retention -> COPD환자 경우 CO 2에 의한 respiratory response↓ -> O 2 필요시보다 고농도로 주면 “hypoxic” drive to ventilation↓ => pa. CO 2 ↑, CO 2 narcosis(ICP 상승, decreased mentality) => 산소를 단계적으로 서서히 낮추어야 한다!! -> Pa. O 2를 60 -65 mm. Hg 사이로 유지하면 발생 ↓ - pul. Vasoconstriction의 redistribution -> ventilation 잘 안되던 곳에서, 잘 되던 곳으로 -> hypoxia 악화

COPD treatment - 기타 1. Prophylaxis - Infulenza vaccination 매년 권장 - Pneumococcal vaccination 5년마다 권장 1. Lung volume Reduction surgery - emphysematous lung tissue resection -> hyperinflation ↓ & exercise capacity ↑ -> Sx. 의 호전 1. Lung transplantation - end stage COPD환자에서 고려

COPD acute exacerbation q q Pa. O 2가 이전보다 10 -15 mm. Hg이상 감소 Arterial p. H < 7. 3 주로 임상양상에 의해 진단 -> dyspnea, wheezing, cough, sputum ↑ 유발인자 – infection(m/c) - 고농도의 산소투여 air pollution LV failure, arrhythmia pneumothorax, pul. thromboembolism sedatives, irritants

COPD acute exacerbation Tx. 1. 2. 3. 1. 유발원인 치료 산소요법 : 최소농도로 시작 -> Pa. O 2 60 -65 mm. Hg(Sa. O 2>90%) 유지 Bronchodilator : short-acting B 2 agonist -> anticholinergics -> theophylline 추가 Oral glucocorticoid : FEV 1증가와 입원기간 감소효과 광범위 항생제 NPPV(noninvasive positive-pr. Ventilation) by nasal mask -> 실패하면 intubation & conventional ventilation시행 Mechanical ventilation : 의식 혼탁, respiratory distress시 내과적 치료에도 불구하고 계속 악화 시 시행
