CONFINED SPACE – MANGAN EMPLOYEES The “non-permit-required confined space” is the only type of confined space our employees are authorized to work in through the authorization of their Project Manager. All aspects of the confined space work are to be documented, mitigated, and discussed through the Project JSA. Further, this type of work falls under our Lone Work guideline and necessitates a ‘Buddy’ to monitor the employee who is working in the confined space to ensure their safety.

The client’s Confined Space program needs to be reviewed and compared to Mangan’s policy to ensure the most stringent guidelines apply.

Go to the company’s medical clinic for a medical evaluation for respiratory use concerning the chemical of potential exposure. 3. Receive a Fit test to guarantee the respirator has a good seal.

INCIDENT REVIEW Long Beach Refinery Client 10/12/12 – Cut Finger, First Aid Injury, Vacuum Tower Worker was loading stainless steel internal packing into a 'skip basket' to be lifted and installed inside the vacuum tower. While loading the packing, he cut the tip of his right middle finger on the metal edge of the packing. The edge did cut through his leather glove. He was taken to the clinic for examination. He received first aid treatment and was returned to work no restrictions. The investigation found that the worker had on the wrong gloves for the job. The JSA for the work identified sharp edges and the potential for cuts and gloves were identified in the PPE section. However, the type of glove was left blank in the JSA.

INCIDENT REVIEW Long Beach Refinery Client In 2012, this client site has experienced 16 injuries to hands and arms. A few statistics about these injuries include: Ø 4 of the 5 recordable injuries have been to hands and arms Ø 5 of the 16 were to client employees, 11 to contractors Ø The injuries have happened to operators, craftsmen and office workers Ø 5 of the injuries happened in the first 6 months of the year. In the 3½ months since, they have experienced 11 injuries to hands and arms Ø 11 of the injuries could have been prevented by using gloves or by using the proper type of glove The last bullet point may be the most important statistic of all. When we permit work and perform work, we need to assure we are using gloves when called for and using the proper type of glove.

Pictured to the right is a leather glove of the type provided by the client. • Leather is good for normal work such as with abrasion hazards like material handling, climbing ladders, using most tools and opening/closing valves. • Leather provides good protection to abrasion. However, leather does not provide much cut protection. To the left are Majestic cut level 3 gloves with Dyneema (a lightweight, extremely tough fiber that is 15 times stronger than steel). • • • Cut resistance level is based on standard tests and ranges from level 0 to level 5. These gloves can feature materials like Dyneema, Kevlar, Hexarmor Super. Frabric and even metal mesh. Generally, the higher the cut resistance, the bulkier and more cumbersome the glove becomes. So, simply specifying a higher cut resistance is not always the right answer to assure the safest completion of the job.

Remember, your hands have no fear. They’ll go wherever your brain tells them to go. • Keep your head and hands in the same place.
