The Risk Assessment (PAR) must be completed and understood by the participants Permit must be authorised by The Responsible Person’s signature to be valid. No entry is allowed without a valid permit. Permits are valid for up to 12 hours. When work is completed, permit and PAR forms should be returned to safety dept. Cancelled permits must be kept on file as required by Policy 21

To control access to the confined space. To summon emergency assistance as needed. To assess hazards in and around the space, and take action on the same. To keep records of confined space work, such as air test results, personnel entry/exit, etc. 29

To immediately exit a space, without question, upon word of the attendant, no matter what the reason. To follow all safety rules and procedures that apply to the job. To be familiar with the work to be performed and the procedures that apply to the job. To use the appropriate PPE whenever necessary. 30

To support the attendant’s authority in controlling access to a confined space. To verify that all personnel have exited prior to closing the space. To ensure that all personnel involved are aware of the hazards associated with the confined space. To ensure that rescue services are available prior to entry. 31

Confined Space Entry SAF 310. 00 Rescue Plans PRIOR to entering a Confined Space: • A proven (tested) Rescue Plan will be in place. • All associated equipment required by the recue plan will have been inspected and made ready. (Tripod, harness, SCBA etc. as applicable) • All members of the work team will be fully conversant with their role in the rescue plan • The OIM/MIC will not permit Confined Space Entry to take place without the rescue plan. 32
