General Information Illinois Nurse Aide Competency Exam • • • You are eligible to take the certification exam. You can take the test at Moraine Valley (zip code 60465) or somewhere else You must register online by the deadline date or you will need to wait until the next month. You can register once your instructor has submitted your final course grade. This should be within 24 hours of your last class/clinical session. Log in during working hours so that if you have difficulties, you can contact Chris Kuropas kuropasc@morainevalley. edu at 708 -974 -5589 or Kelli Nickols nickolsk@morainevalley. edu at 708 -974 -5585 for support with issues logging in. The testing fee has been included in your tuition payment so a voucher number has been provided for you. Use that number including all zeroes. You must enter your Moraine Valley email, social security number and date of birth into the website to check your eligibility to take the exam. If you have difficulty logging in, try over and over again to be sure that you are entering it correctly. It must match your information sheet exactly. You must create a log in and password. Use only letters & numbers, no symbols. Do not use your email as a password. Make sure to record the password you created, as you will need to remember it when taking the state exam. Complete the application process and enter your voucher number. A receipt will appear. On the bottom of the receipt is the date, time and location of the test. Print the receipt and bring it with you on the day of testing along with a photo ID and 2 #2 pencils. You must also know your social security number to take the exam.

To Receive Your Test Results, use these instructions Check the Health Care Worker Registry on the following Friday after 4 PM. https: //hcwrpub. dph. illinois. gov • In the middle of the page, go to “search the health care worker registry, ” enter your first & last name and hit search. • Choose your name & hit the icon. Your results will appear. • At the bottom, under competency, a “P” means that you have passed. An “F 1” means you have failed for the first time. • You can print the page as verification of your results for employers. • To print a certificate, log into the online registration program & click on the certificate link which appears in the gray box. • To reschedule a failed test, log into the online registration program 2 weeks after your previous test date and choose “schedule a retest” or click on the “store” button at the top. • Student who are unable to appear for a test on the test date, can reschedule their test using the online program. If known in advance, another test can be scheduled for the following month, if done before the registration deadline. If the registration deadline has passed, rescheduling can be done but will skip a month.

For example, the password can not be knickols@moraine. com • Password is caps sensitive, no symbols, and keep the password simple so that you can remember it. • You should take the test at the provided testing date. • Your results will be available usually within 1 week of testing.
