What is Extra Care? • Working with housing associations and local authorities, we provide care and support in purpose-built accommodation for older people, offering a home-from-home environment, with support on-hand when needed • access to a wide range of facilities, services and activities, aimed at supporting the individual to maintain a great quality of life, as well as improving their health and wellbeing.

What do we do? • Following an initial assessment, the Individual and the Service

What do we do? • Following an initial assessment, the Individual and the Service Leader/ Senior Support worker will compile a support plan based on their needs. It is important to include everybody who is important to them to create an effective Person centred Support Plan • The support plan will continue to be developed as the Individual changes & relationships develop within the service or when their needs and preferences change. • Everybody including all the staff team have a One Page Profile which enables us to match staff to service users & also create opportunities for activities Pendant alarms worn by the individual & also wall units in apartments & communal area are keeping individuals safe within the site without taking away their independence.

John from Derby Court is studying History using a tablet provided by the scheme. John has always enjoyed exotic holidays abroad & is currently plotting all the historical landscapes he still wants to visit. His One page Profile enabled us to identify his interests & goals whilst his support plan & risk assessment allows us to be realistic whilst ensuring he is able to do all the things he wants to do.

We Skill match our team to ensure we deliver the best care, Working with individuals with Dementia we have ensured that all of our team attend regular training & we are currently supporting all our Support Workers to become Dementia Friends. Prior to moving into Crawford Gardens this gentleman was isolated in his own home with his family terrified that he would leave his home & either get lost in the community or have an accident. The team have worked with him & his family to gain a greater insight into history & his likes & dislikes. He enjoys lunch daily in the Bistro where both the Bistro staff & the support workers work in unison to ensure he has a balanced & varied diet. He walks to the Salon with his own money to get a trim & staff now support him to walk to the paper shop every Sunday just like he did pre his Dementia deteriorating. He also walks another tenants Dog every evening around the garden. KHT staff have supported us to get a postbag & ensure we have internal post for him to deliver reflecting on his previous occupation as a postman.

The relationship with the tenants & the staff within both Extra Care Sites is

The relationship with the tenants & the staff within both Extra Care Sites is built upon the effective partnership working with the housing provider, the commissioners, ourselves & the tenants. Clear boundaries & expectations have been set out from day one & as the service grows & the tenants needs change it is vital that we continue to work together to deliver the same goal. In Crawford Gardens we have delivered such a range of care within the last 12 months including End of Life Care. KHT supported us to ensure that this was done empathetically & with dignity. We were able to bring Christmas forward for one lady & her Husband & the support given to her husband after she had passed away enabled him to carry on & to know that all the staff on the site for would support him. Extra care despite its enormity does have a real sense of community……………………. It’s a village with a really big roof!
