History of Practice Pharmacists • These have existed for over ten years but this has been given a massive boost since 2015 with the NHSE pilot (aided by qualification as NMP's) • This pilot has now been extended (? evidence) with the aim of there being a PBP for every 30, 000 pts
What can we do? • Improve prescribing, management of LTCs, medicines reconciliation, complex polypharmacy, de-prescribing In order to improve health outcomes, reduce admissions, medication errors, drug expenditure, communication across sectors.
Evidence of Benefits • Reductions in Hb. A 1 c • Improved BP control • Better anti-coagulation management
A Typical Day • Assisting with Telephone triage – meds / minor ailments / infections / hayfever / GIT • Medication reviews including care homes • Prescription queries • Hospital letters • BP/Asthma reviews • Helping other professionals / Staff
The Future? • Increasing complexity and diversity of therapies e. g. biologicals • More high tech treatments based in the community • Ageing population, increasing care home residents • Specialist clinics
