Ms. A. B. is a caring and compassionate Student Nurse. Unfortunately, Ms. A. B.

Ms. A. B. is a caring and compassionate Student Nurse. Unfortunately, Ms. A. B. has had difficulty arriving to the clinical facility in a timely manner and when she does arrive she is often unprepared for her clinical assignment. Many times Ms. A. B. arrives without her equipment and patient plan of care. During a recent medication pass with A. B. , patient C. D. was noted gasping for air in her bed and appeared to be in significant respiratory distress. She was unable to call for assistance because her call bell was not within reach. After a rapid response was called, the patient’s respiratory status was brought under control. When the Clinical Instructor discussed the patient’s status with Ms. A. B. , Ms. A. B. appeared to be unaware of the patient’s history and events from the previous shift, where it was noted that the patient has had exacerbations of asthma attacks. When the Clinical Instructor further discussed this situation with Ms. A. B. , Ms. A. B. stated, “the nurse could not give her report because she was busy. ” When the instructor asked specific questions regarding the patient’s respiratory status prior to this episode, the student stated she forgot to assess lung sounds and never completed the initial assessment because she was waiting at the nurse’s station for the nurse to come and give her report. At the end of the clinical day and after a lengthy discussion regarding the student’s tardiness and lack of preparedness for clinical, the student was crying and stated she knows she is not as prepared as she should be for her weekly clinical assignment. She is in a “bad” relationship and is having financial difficulties which has caused her to work long and late hours, leaving little time for school work. Regrettably, due to lack of regular preparedness for clinical assignments and several clinical patient safety factors that included unfamiliarity with the patient’s history, incomplete initial patient assessment, and not placing the call bell within the patient’s reach, the student will not pass this clinical rotation. The student is encouraged to seek help from her advisor, as well as assistance from financial aid, to ease financial burdens related to being a full-time student. She is also encouraged to discuss her personal situations with a counselor in the wellness center, with the hope that when she re-enters the nursing program she is in a more stable personal and financial position to be successful.
