Risk Assessment: Cardiologist ● Clementine (age 34) has at least a 20 percent chance of having a heart attack in the next ten years ● Last week she had a transient ischemic heart attack (mini-stroke) ● She’s a smoker (1 pack/day) ● Her total cholesterol levels are elevated ● Her family has a history of strokes (grandmother died of a stroke) ● She works a lot and is stressed out ● She does not sleep much
Lifestyle Changes: Dietician ● Ceasing to smoke would greatly decrease Clementine’s risk of having a heart attack ● Following a diet lower in lipids, but higher in protein, fiber, and unprocessed foods ● Clementine should follow a more regular sleep and exercise plan ● Clementine should try to get a job with fewer hours and find time to relax
Plan to Achieve Health 1. Stop smoking. Gradually decrease-½ pack, then one a day, then none over three weeks. 2. Meet with a dietician every three weeks. Make a plan to follow a diet higher in unprocessed, healthy foods-no empty Calories! 3. Meet with a physical trainer three times per week to get in a good workout. 4. Work with your boss to decrease your hours. Provide a doctor’s note if necessary.
Medication/Medical Treatment Plan ● Undergo an EKG to assess blockage in the heart. ● If there is some blockage, treatment would include Statin medication. If there’s significant blockage, an angiogram with stent placement may be a viable option ● Take ACE inhibitors to lower systolic blood pressure, as prescribed.
Goals: (Support from: Nurse, physical trainer, dietician) ● ● Blood pressure goal: 120/60 (down from 137/91) Total cholesterol goal: less than 200 Clementine will undergo blood testing every four months Gaining of 10 pounds, via proteins and unprocessed, healthy food. A specific goal for BMI (body mass index) will be determined by the dietician, taking into account BMR (basal metabolic rate) and planned exercise routine. ● Clementine will come in for check-ups every three months. Her blood pressure, weight and BMI will be measured and kept track of by the nurse. ● Increased physical fitness with regular, guided exercise ● Clementine will undergo a stress test after two months of guided exercise
