Fund Purpose AFHS partners with nonprofit agencies to support safety net and human service programs serving young children, youth, immigrants, persons with low income, seniors and persons with disabilities. 2

FY 2019 – FY 2021 Priorities Grant awards are made for proposals that address 1 of the following 5 outcome areas: § School and career readiness for children and youth § Social connectedness, emotional security and culturally competence for children and youth § Economic security for individuals, families and seniors § Access to health and mental health resources for Individuals, families and seniors § Preventing and remedying crises for individuals, families and seniors 3

Fund Facts § Contingent on City Council approval AFHS awards will provide funding for a three year grant cycle; July 1, 2018 – June 30, 2021. § Current AFHS allocations FY 2016 – 2018 $1, 996, 430 annually. § AFHS grants support programs/services provided by orga nizations based inthe City of Alex andria or in other jurisdictions, but funding can only be used to provide services to Alex andria residents. § 47 programs were awarded for FY 16 – FY 18. 4

Online Application, cont’d § Proposals must be transmitted electronically using online application by 11: 59 p. m. Monday, February 12, 2018. § Proposals delivered in person, by U. S. Postal Services, courier, email or facsimile will not be accepted. § Incomplete applications will not be considered. § Successful applications will receive an official notification of award by May 24, 2018. 6

Post Award § If the award amount is not as requested, grant recipients will be given an opportunity to revise outcomes and budget If revisions are not made, the expectation is that recipients will meet requirements as initially proposed § For each Fiscal Year, grantees are required to submit an interim and a cumulative final report, and/or an annual audit, year end financial statement, or IRS 990 § Upon review and approval of each biannual report, grantees will submit invoices to AFHS staff for processing 8

Post Award cont’d. § Grant recipients are expected to retain all records relative to the grant for a maximum of 3 years after final payment is made § City’s authorized agents must have full access to and the right to examine any programmatic or financial records specifically related to the grant for the same period of time § City reserves the right to discontinue funding if it is not satisfied with the grant progress in the event of discontinuation or closure, any unexpended funds must be immediately returned to the City. 9
