CHNA Reporting Requirements – Impact of Programs Tax-exempt hospitals must include in their current CHNA report an evaluation of the impact of programs or activities taken to address significant health needs identified in the immediately preceding CHNA. Outcome Measures Outcomes (or impact) measures focus on what changed as a result of the program and whether the program achieved goals: Short Term Result Learning Awareness Knowledge Attitudes Skills Opinions Aspirations Motivations Medium Term Results Action Behavior Practice Decision Making Policies Social Action Long Term Results Conditions Social Economic Civic Environmental Note: Long-term measures will be captured by the Adventist Health System Office 9

Impact Examples – Which one tells a better story? Process Related Measures 50 students

Impact Examples – Which one tells a better story? Process Related Measures 50 students (process) and parents participated in a 4 -part asthma education program and 95% found the information helpful (process). Outcomes Related Measures The 50 students (process) and parents who participated in the asthma education program increased their knowledge (short term) about asthma and how to control it. The students participating in the program experienced significantly fewer school absences (medium term). While their ED use did not decrease in the first year, it did decrease significantly in years two and three after changes were made to the program to increase parent participation (medium term). 10
