Changes to the SUD Treatment Plan (effective 11/4/19) Substance Use Disorder (SUD) Providers October 2019
Reminders • Effective November 4, 2019 • Initial and Updated treatment plans shall be submitted using the SUD Treatment Plan form in Avatar • Staff will not be required to re-submit treatment plans previously completed in Avatar (prior to November 4 th, 2019) • Staff may access historical treatment plans submitted in Avatar prior to November 4 th, 2019 via the 7002 SUD Treatment Plan report.

SUD Episodic Treatment Plan Changes 1. Plan Name: Plan Name will be auto populated in Avatar. Staff are asked to please not change the Plan Name (this could affect timeframes/audits). 2. Plan Date: The start date of the initial or updated treatment plan. 3. Next Review date: Date the team member will be reminded to review plan with client to begin updates 4. Plan End Date: The end date of the plan 5. Treatment Plan Status: All SUD treatment plans are required to be finalized once completed and all required signatures have been obtained.

SUD Episodic Treatment Plan Changes Required signatures will now be including within the actual treatment plan and required for each new/updated treatment plan. The “Treatment Plan Participation Consent” Form will no longer be accessible or required. Staff may access historical treatment plan participation consents submitted in Avatar prior to November 4 th, 2019 via the 109 Tx Plan Participation Consent Report.
