The Mental Health and Intellectual Disability Act of 1966 � The local authorities of each county separately or in concert with another county or counties, as the secretary may approve, shall establish a county mental health & intellectual disability program for the prevention of mental disability, and for the diagnosis, care, treatment, rehabilitation and detention of the mentally disabled and shall have the power to make appropriations for such purposes. Centre County Continuum of Services �Services listed below include those that are provided by contracts the County holds with providers and receives federal, state, or local funding based on contract budget. �Services include those required through the Act and State Plan. Additional services are outlined in Resource Book. �Private providers are located throughout the county through health care insurance providers in addition to those listed below.
Centre County Continuum of Services �Voluntary Inpatient Hospitalization - 201 ◦ Individuals seeking voluntary inpatient hospitalization in Centre County can seek assistance from Emergency Department (ED) at Mount Nittany Medical Center or by Meadows Psychiatric Center. ◦ Facilities outside of Centre County can be linked if seeking support from ED or Can Help call. ◦ Insurance or self pay is necessary for this service.
Centre County Continuum of Services �Involuntary Commitment Inpatient Hospitalization -302 � An involuntary commitment is an application for emergency evaluation and treatment for persons who present a significant danger to themselves or others due to a serious mental illness. �An involuntary commitment is for up to 120 hours of psychiatric inpatient treatment.
Centre County Continuum of Services �Mental Health Procedures Act outlines the necessary obligations of the county and Physician to determine an involuntary commitment. �County contracts with Can Help to provide Delegate responsibilities of the Involuntary Commitment process including issuing warrant, securing belongings, and bed search if person is determined to need treatment.
Centre County Continuum of Services �Emergency Services �Mobile Crisis: Phone, Walk in And Mobile Response available 365/24/7. ◦ Can Help operates the service 1 -800 -6435432. ◦ Individuals, providers, community members, etc. can contact Can Help for support. ◦ Information pertaining to services, supports, and follow up are provided.
Centre County Continuum of Services �Intake Procedures for County Services ◦ Individuals seeking mental health services can call Centre County MH/ID EI D&A Office at 814 -355 -6786 and speak to intake worker. ◦ Based on income, insurance, and diagnostic eligibility, we can assist with connecting individuals to our services. ◦ We assist individuals with information and referral if public or private health care insurance providers can assist in payment of services.
Centre County Continuum of Services �Centre County Mental Health is currently serving 888 individuals. ◦ Majority of individuals are receiving Case Management. �Blended Case Management: Individuals seen as often as weekly to monthly based on needs. Children & Adults �Strawberry Fields also offers Blended Case Management Services for children and adults �Primarily see individuals in the community, whether their home, provider site, and/or community meetings. �Administrative Case Management requires annual visit for eligibility. Individuals are receiving outpatient services the county is funding and minimal further case management is needed.

Centre County Continuum of Services �Psychiatric Rehabilitation ◦ County provided service to 34 individuals during Fiscal Year 2017 -2018. ◦ County converted to the model of Psychiatric Rehabilitation in the early 2000 s. This transition to a whole person, recovery based service was at the request of our individuals seeking this model rather than the traditional treatment model of partial hospitalization. ◦ Skills, Inc. ◦ Community Services Group
Centre County Continuum of Services � Community Employment and Vocational Rehabilitation ◦ County provided service to 16 individuals during Fiscal Year 2017 -2018 ◦ Skills, Inc. provides community employment and vocational rehabilitation services ◦ Opportunity Centre Clubhouse – Transitional Employment Program ◦ Mobile Psychiatric Rehabilitation ◦ ID-MH Employment Workgroup to list employment supports. � Consultation and Education ◦ Penn State Psychological Clinic provides consultation and education across several opportunities to include our Correctional Facility.
Centre County Continuum of Services �Centre County has 7 beds allocated at Danville State Hospital for our residents. Danville has a total of 161 beds at their facility. �Danville Hospital serves 17 counties in their area to provide inpatient long term psychiatric care. � CHIPP: Community Hospital Integration Project Program. This funding from the State is available at times to support community enhancements for individuals.
Centre County Continuum of Services �Aftercare Services for Persons released from State and County Facilities �Individuals released from Danville State Hospital have the option to explore a variety of housing and supportive services based on choice. �Centre County has expanded these services to include individuals who could be eligible for Danville, be diverted to a more supportive living environment, thus not having to require a state hospitalization.
Centre County Continuum of Services �Community Residential Services/Housing Supports ◦ Community Services Group ◦ Strawberry Fields ◦ Eagle View/Eagle Valley �County provided service to 65 individuals during Fiscal Year 2017 -2018 �Peer Supports – Required in the State Plan ◦ Cen-Clear and Strawberry Fields, Inc. provide Peer Services ◦ Certified Peer Specialists receive training and work to engage with individuals in the community.
