How to monitor use of CD-JEV (1/2) l Immunization cards, tally sheets, stock forms and monthly reporting forms are being updated to include JE vaccine. l Record JE vaccine doses given on the child’s immunization card and the health center or clinic recording register. How to monitor use of CD-JEV (2/2) l Keep a record of doses used during the immunization session and then enter the total doses used on the routine reporting form at the end of the session. l It is important to monitor the use of vaccine and to think about ways to reduce wastage. Check used vaccine vials to ensure reconstitution is done well so that the full number of doses are used (for 5 -dose vials) each time. l Recording and reporting should also include completion of vaccine stock management forms to ensure sufficient supplies are available at the vaccination sites. 6 | Recording and monitoring uptake of JE vaccine, Module 5 | September 2020
