How to monitor use of CD-JEV (1/2) l Immunization cards, tally sheets, stock forms and monthly reporting forms are being updated to include JE vaccine. l Record JE vaccine doses given on the child’s immunization card and the health center or clinic recording register. How to monitor use of CD-JEV (2/2) l Keep a record of doses used during the immunization session and then enter the total doses used on the routine reporting form at the end of the session. l It is important to monitor the use of vaccine and to think about ways to reduce wastage. Check used vaccine vials to ensure reconstitution is done well so that the full number of doses are used (for 5 -dose vials) each time. l Recording and reporting should also include completion of vaccine stock management forms to ensure sufficient supplies are available at the vaccination sites. 6 | Recording and monitoring uptake of JE vaccine, Module 5 | September 2020
How to record CD-JEV on the monthly report l Report CD-JEV doses given each month, along with other vaccine doses. l Use tally sheets to prepare monthly reports to send to supervisors. l If you have old reporting forms, add lines to report JE vaccine doses given. 7 | Recording and monitoring uptake of JE vaccine, Module 5 | September 2020
How to identify eligible infants and inform parents/caregivers about JE vaccination l Inform parents of the importance of protecting their child against JE. l Explain that JE vaccination is recommended for children 8 months of age and older for maximum protection against severe JE. l If too young to be eligible at the visit, remind parents when the child should return for JE vaccination. l If possible, follow up with the parents of children who are due for vaccination but have not yet come to the health center. 8 | Recording and monitoring uptake of JE vaccine, Module 5 | September 2020

Key messages l Fill in the immunization card once the JE vaccine has been administered to the child; if the parent has brought an old immunization card, either write in the JE vaccination data by hand on the old card, or transfer ALL immunization data for the child onto a new card. l The tally sheet, vaccine stock management form and monthly report forms should all be updated to include JE vaccine. l Explain to parents the importance of JE vaccination and remind them that vaccination is recommended at 8 months of age, if their child is not yet eligible. l Track monthly targets for JE vaccination closely; be sure to follow up regularly with parents of eligible children. 10 | Recording and monitoring uptake of JE vaccine, Module 5 | September 2020
